Regarding the referenced project, construction has been completed and on behalf of the Owner, I
am requesting an operating permit; please note the following:

1. The project was completed according to approved plans and conditions. Any variation
from the plans were made in accordance with Utah State Code.
2. One set of construction record drawings have been given to the Owner and one set
was sent to your office previously.
