To Whom It May Concern,
The following serves as a final report detailing environmental cleanup and remediation activities
for the incident that occurred at the above referenced location on23 Sep 24. Please see the site
map attached (Appendix l).
Incident Background:
On 23 Sep 24, at 0604 ET, Penske contacted ERTS to report a 75-gallon release of diesel fuel
onto asphalt due to mechanical failure. The spill site is EB on I-80 at MM 184 in Emory, UT. It
was later determined that approximately SO-gallons were released to the asphalt roadway and
adjacent soil. It was also determined this was due to the truck hitting debris in the roadway.:'
Emergency Response:
On 23 Sep 24, Penske retained Emergency Response and Training Solutions (ERTS) to manage
and oversee environmental operations. ERTS'dispatched EnviroServe out of Salt Lhke'City, UT
to conduct cleanup operations.

Corrective Action:
On 23 Sep 24, at 1200 ET, EnviroServe arrived on site and assessed the impact areas.
EnviroServe confirmed that 50-gallons of diesel fuel were released to the asphalt shoulder
measuring 150ft by 3ft. There was the adjacent soil impact, and it measured 30ft by 30ft. The
asphalt impact was treated by applying granular absorbent, working into the surface, and
sweeping the waste into a drum. The emergency crew placed a pool under the truck to capture the
diesel. EnviroServe vacuumed the diesel and placed it into a drum. The soil impact was secured
for later excavation. One drum of solid rvaste and one drum of liquid waste were removed from
the site on this day. The impacted soil area was already marked and flagged off for remediation
by Nate Brooks, 435-659-9568, with the Environmental Health Director Summit County.
On 14 Jan 25, The contractor contacted ERTS indicating they were onsite and would begin
remediation once the barriers were in place. The area needed excavation was 20ft by 30ft and did
not speciff the depth at that time. Using a mini excavator Enviroserve removed an estimated25
to 30 yards of impacted soil. At 1800 ET, Enviroserve advised they will be departing the site and
returning tomorrow to complete the backfill.
On I 5 Jan 25, Enviroserve returned to the site before starting the backfilling and pulled four
samples to be analyzed. Please see provided Analytical Results in (Appendix 2). The crew
continued to backfill and bring the area back to its original condition. All on-site work was
completed on l5 Jan. All waste was removed from the site and stored at the contractor's shop
until profiled and proper disposal. Please see the attached provided site photos (Appendix 3).
Waste Disposal:
On 0l Oct 24,Enviroserve transported two 55-gallon drums, one with solids and one with liquids
to Sump &.Trap Cleaning in Salt Lake City, UT and between 10 Feb and 1l Feb a total of 40.34
tons of impacted solids to Wasatch Regional Landfill in Skull Valley, UT for proper disposal
Please see the attached disposal documents (Appendix 4).
Conclusion and Recommendations:
The release has been completely remediated and no evidence was found that suggests any
significant environmental impact remains on the site. ERTS suggests that this project be closed,
and no further remedial action be taken.
