In accordance with 40 CFR 63.1206(c)(3)(vi)(A) and Title V Condition II.B.2j, Clean Harbors
Aragonite, LLC is required to self-report each set of ten exceedances of an emission standard or
operating requirement while hazardous waste remains in the combustion chamber during a 60-
day block period. An automatic waste feed cut-off occurred immediately for each of these events
and stayed in place until the emission standard or operating requirement was reestablished.
Investigations into the events immediately followed each occurrence and corrective actions were
implemented. Table I lists each occurrences date, time, duration, type of exceedance, cause and
corrective actions that were implemented.
