Salt Lake City, UT 84104 Facility ID #4002610; Release OMV Dear Mr. Crim: The enclosed report by Maverik, Inc (Maverik) summarizes a diesel fuel surface spill and associated cleanup response that occurred on 3, 2025, at our Salt Lake City #669 store reference above. Maverik #669 Site Details Maverik’s store #669 is composed of a convenience store, three 25,000-gallon underground storage tanks (one 25,000-gallon diesel and two compartment tanks – 13K Unleaded/12K E15 and 8K Ethanol Free/9K Premium/ 8K DEF), and two fuel canopies (auto and commercial). The commercial fueling area is equipped with high flow diesel dispenser pumps. Surface drainage features include concrete pad at top of tanks, product piping runs, and fueling/canopy areas; asphalt surface pavement throughout the site; onsite (private) gutter inlets with catch basins, drainage piping, and an onsite detention chamber. Figure 1. Site map of the facility and spill details Spill Event and Response The diesel spill occurred on the morning of March 3, 2025, and was estimated to be 25-30 gallons. This spill was likely caused by a faulty auto shut-off mechanism at pump #22, and made worse by the fact the customer was not attending the nozzle during fueling. Store personnel, with the help of the local fire department, responded by containing the diesel spill so that the nearest stormwater inlet would not be impacted. They were able to stop the spill from impacting the inlet but did not have adequate cleanup material to finish cleaning up the majority of the spill on the asphalt surface pavement. Maverik’s emergency response contractor (Environmental Restoration, LLC) was dispatched to assist in the spill cleanup response. Store personnel, Fire Department personnel, and ERLLC personnel utilized granular absorbents and Coco-absorb to contain, and cleanup petroleum found on the concrete and asphalt surface pavement. The surface spill did not impact any soil or stormwater inlets/catch basins. Once the absorbent materials were swept up, the entire spill area was pressure washed. Impacted water from the pressure washing event was vacuumed up and containerized without the water getting into the stormwater inlets. All wastes were containerized into drums and later taken offsite to Sump and Trap Cleaning in Salt Lake City for appropriate disposal (see the attached ERLLC Spill Response Notes, Waste Manifest, and Photo Documentation). Conclusions Based on a review of the incident and in consultation with the store personnel and spill responders, all spilled product was contained and cleaned up at the site. No impacts to soil, groundwater, public storm drainage, or surface water occurred. All cleaned up material was containerized and hauled offsite for appropriate disposal. As such, Maverik requests that the DERR consider closure of this surface release and issue a No Further Action (NFA) letter.. The following information was provided at time of request: Released Product: Gasoline or Diesel Cause of Released: Fuel Release Amount Released: TBD (select units) Site Location: Maverik - 669 5600 West 300 South Salt Lake City, UT 84104 On Site Contact: Keana MOD Scope of Work: Remediate all impacted surfaces and destination for the generated material will be determined when the team identifies what kind of fuel has released. Upon receiving the response request, Environmental Restoration, LLC immediately began coordinating the appropriate personnel and equipment regarding the above referenced project. 0959hrs Environmental Restoration, LLC (ER) is contacted by Maverik representative, Mr. Mark Stewart, regarding the above- mentioned project. Mr. Stewart reports Maverik store 669 has release of fuel on the pavement. Initially the onsite personnel cannot tell ER if the product is diesel or gasoline or any discernable quantity. Mr. Stewart then requests for a team to respond to the above referenced location as soon as possible to provide labor, equipment, and materials to remediate the release. Any generated material will be properly containerized, labeled, and the generated material will be taken off site for disposal by ER personnel. 1005hrs ER creates the Authorization to Proceed as well as all other electronic documentation associated with this project. ER then informs Mr. Stewart the team will be onsite within one hour. 1045hrs Team members arrive onsite and meet with the manager on duty (MOD). The team is briefed with the incident details and directed to the incident area. A site-specific Health and Safety Meeting (HASP) is conducted, and a scope of work is established. 1050hrs The crew dons the proper PPE and begins the plan of action for this project. Mr. Stewart is then contacted and informed the team is on-site and establishing the scope of work. The RM reports the release consists of an estimated 40-gallons released onto the pavement. The onsite personnel applied granular absorbents prior to the team's arrival onsite which prevented any of the diesel from entering any drains or waterways. The team will apply granular absorbents and work them in with hand tools to solidify the product. Once the team has solidified the product they will containerize it. Once the spent absorbents are containerized the team will apply a degreaser to the surface area the release occurred and then begin to power wash the area. 1145hrs The RM reports the team members have applied granular absorbents to the released diesel and are working them in with hand tools. Once the diesel has been absorbed and solidified the team will utilize hand tools to containerize the absorbents. The degreaser will be applied utilizing a sprayer and then the team will pressure wash the impacted surfaces and ensure all the release has been completely remediated. 1666 Fabick Drive Fenton, MO 63026 (636) 227-7477 Providing Quality Environmental Remediation, Temporary Staffing and Emergency Response Services Toll Free 24-hour Emergency Response 888-814-7477 1300hrs The team members have applied the granular absorbent to the impacted areas and have worked them in with hand tools. The team utilizes hand tools to remove the spent absorbents and containerize them into 55-gallon steel open top drums. The team will now apply degreaser to the area and begin pressure washing directly after the degreaser. 1430hrs The RM states the team has completed the process of applying the degreaser to the impacted area and are currently pressure washing the impacted area. Team members are currently utilizing the pressure washer and a drum vac to extract the impacted water being produced by the pressure washing. 1530hrs Team members continue to pressure wash and utilize a drum vac for extraction of the impacted water produced by the pressure washer. Team members have containerized all spent absorbent into a 55-gallon steel open top drum and containerized the absorbent boom previously applied by on site personnel. Team members will continue to pressure wash until the impacted surface is fully remediated. 1620hrs ER notes that all mitigation efforts have concluded, and no injuries occurred during this project. ER then updates Mr. Stewart with the notice of project completion. The team members generated three 55-gallon steel open top drums containing spent absorbents, absorbent boom, debris and PPE. The project also generated four 55-gallon steel drums containing impacted liquid. All generated material will be staged onsite pending profile and then will be picked up and taken off site for disposal. 1630hrs The team demobilizes from site and are enroute to refuel equipment and return to their facility. Upon arrival at the facility, the team will restock supplies, stage equipment, and complete all project paperwork. Tuesday March 4, 2025 1000hrs-1130hrs The team arrived back onsite at the Maverik location 669 to pick up the seven drums of generated material today and took it to Sump & Trap disposal facility for disposal. The project in total lasted from 1000hrs to 1130hrs.
