This letter certifies that the surface seal of the subject well was applied with materials and techniques which comply with the applicable portions of Utah Administrative Code (UAC), Rule R309-515-6(6)(i), Grouting Techniques and Requirements, and Rule R655-4-11.4, Surface Seals and Interval Seals. Background Information of the Well The Rainbow Cove Well is being drilled under water right # 23-3759 (a53205) and is owned by Rainbow Cove LLC. The well is intended to serve as a public water supply well for a proposed residential development in unincorporated Rich County. Cascade Water Resources is the design firm for the well. The Utah Division of Drinking Water (DDW) well drilling plan approval letter associates the system with the Rich County General System #1700, File #22151, and assigns the well water source #WST01. Well Location Description The Rainbow Cove Well is located on the east side of Bear Lake, approximately 12 miles north of Laketown, west of Cisco Road near the UT/ID border in unincorporated Rich County. The location of the well was determined by a handheld GPS unit to be UTM 12 T N: 4648039 E: 0477613. The well head elevation is approximately 5941 ft. Well Driller Information The well is being drilled by Lang Drilling (Utah Well Driller’s License #568). Kyle Schilling and Costner Eborn are the drill rig operators. Lang Drilling’s mailing address is P.O. Box 27558, Salt Lake City, UT, 84127-0558. Their phone number is 801-294-5178. Well Construction Information A 20” diameter borehole was drilled with a reverse rotary rig to a depth of 117 feet below ground surface (bgs) and a 16” diameter carbon steel casing was installed from 117 feet bgs to approximately 2.5 feet above the ground surface. Subsequent well drilling will include drilling a borehole diameter sufficient to accommodate 6” well casing and gravel pack to approximately 300 feet bgs. It is unknown if a pitless unit or adapter will be installed. Well Sealing Description The neat cement grout placement began at 7:13 p.m. on Wednesday July 18th. Neat cement grout was mixed on site using a hopper and grout mix pump. Neat cement super sacks were utilized with a weight www.jub.com J-U-B ENGINEERS, Inc. between 2,100 and 2,300 pounds. The neat cement grout was placed with a 1.5” tremie pipe located in the annular space between the 20” borehole and 16” well casing. Sections of 20 ft tremie pipe were removed as the grout was placed. The bottom of the tremie pipe was maintained below the grout surface. The neat cement quantity on hand at the beginning of the grouting operation was 1.9 times the calculated volume. At 9:26 p.m. the last of the grout was placed. The grout level was tagged at approximately 25 feet bgs. Additional cement was delivered to the site the following day and grouting resumed at 12:29 p.m. on Thursday July 17th and concluded at 12:45 p.m. with the grout being placed to the surface. The density of each batch of cement grout was measured to verify minimum density requirements were met prior to placement. Batch densities ranged from 15.0 to 16.1 lbs./gal with an average density of all batches of 15.4 lbs./gal. The calculated volume of the annular space is 91.9 c.f. or 3.4 c.y. Seven total batches of neat cement grout were pumped into the annular space to the surface. The grouting operation concluded at 12:45 p.m. on Thursday. The total volume of neat cement grout placed was approximately 198.5 c.f., or 7.4 c.y. yielding a volume ratio of placed to calculated of 2.2. A copy of the letter from DDW authorizing me to perform the well seal witness is attached, along with some photographs and a figure of the approximate well location.
