Dear Mr. Bird,
In accordance with condition I.S.2 of Clean Harbors Aragonite, LLC (CHA) Title V Operating
Permit and Utah Administrative Code (UAC) R307-415-6a(3)(c)(i), CHA is submitting the
Semi-Annual Monitoring Report for May 2023 through October 2023.
CHA had no deviations from Title V permit requirements during this time period.
ln accordance with Permit Condition I.K and Utah Administrative Code R307-415-5d, I certifr
under penalty of law that this document and all attachments were prepared under my direct
supervision in accordance with a system designed to assure that qualified personnel properly
gather and evaluate the information submitted. Based upon my inquiry of persons or persons who
manage the system, or those persons directly responsible for gathering the information submitted
is, to the best of my knowledge and belief, true, accurate and complete. I am aware that there are
significant penalties for submitting false information, including the possibility of imprisonment
for known violations.
