In accordance with condition I.S.2 of Clean Harbors Aragonite, LLC (CHA) Title V Operating
Permit and Utah Administrative Code (UAC) R307-415-6a(3)(c)(i), CHA is submitting the
Semi-Annual Monitoring Report for November 2022 through April2023.
During this time period, CHA self-reported one deviation:
1. CHA failed to properly change out a carbon bed within its set run hours as required by
condition II.B.4.a.l(e) and II.B.6.a.l(b). Notice of this error was self-reported by the
facility on January 10, 2023. This error was discovered by facility personnel during a
compliance review on January 5,2023.I1was caused by an operator error in not properly
updating the operating system when the new carbon bed run hours were determined. This
was immediately corrected and to prevent the issue from reoccurring a documented
quality check was implemented to ensure programing is properly changed.
In accordance with Permit Condition I.K and Utah Administrative Code R307-415-5d, I certiff
under penalty of law that this document and all attachments were prepared under my direct
supervision in accordance with a system designed to assure that qualified personnel properly
gather and evaluate the information submitted. Based upon my inquiry of persons or personJwho
manage the system, or those persons directly responsible for gathering the information submitted
is, to the best of my knowledge and belief, true, accurate and complete. I am aware that there are
significant penalties for submitting false information, including the possibility of imprisonment
for known violations.
