Rule: R309-545-14(2)Rule requires the access opening to a tank to be furnished with a gasket between the lid
and frame. This significant deficiency must be corrected within 120 days of notification
or have a corrective action plan approved by DDW.
Notes:Tank lid has a gasket but needs to have a thicker one.

Rule: R309-545-6(1) and 545-9 Rule requires openings in a storage tank roof or top, designed to accommodate control
apparatus or pump columns, to be welded, gasketed, or curbed and sleeved and to have
additional proper shielding to prevent vandalism. This significant deficiency must be
corrected within 120 days of notification or have a corrective action plan approved by
DDW.
Notes:Conduit needs to be repaired. The joints are broken and has created openings into the tank.
