Subject: Groundwater Quality Discharge Permit Number UGW450005 (GWQDP)
Notification and Resolution of BAT Concern: Manhole 2 Moisture Sensor
Dear Mr. Hansen:
On June 10, 2025 the BAT inspector discovered that the moisture sensor in Manhole 2
could not be verified to be operational during routine inspections. Clive Maintenance
personnel were notified the same day to repair the sensor.
On June 13, 2025, the moisture sensor was replaced, the system was tested, and normal
operations were restored.
EnergySolutions provided verbal notification of the moisture sensor failure to the
Division of Waste Management and Radiation Control (DWMRC) via e-mail on June 11,
2025. This letter provides the written notification required within 7 days of discovery by
GWQDP Appendix K Part 4.16.2 for the East Side Drainage System - Manhole 2.
