Subject: Radioactive Material License UT 2300249 Follow Up to Notification of Discrepancy with 0631-01-0001 Dear Mr. Hansen,

In a letter dated June 10, 2025 (CD-2025-121), EnergySolutions notified the Director of the Utah Division of Waste Management and Radiation Control (DWMRC) of a discrepancy with Shipment 0631-01-0001 (Bates Number L141165) (GSAP #2411000509). Follow up to that notification is provided herein.

Condition Report # CR-2025-0636 has been written to track resolution of this issue. Shipment 0631-01-0001 arrived at the Clive Facility on June 5, 2025. During sampling on June 9, 2025, in accordance with Attachment II-1, Waste Analysis Plan, of the state-issued Part B Permit, liquids greater than 1% were observed in 7 of the 47 containers. Included with this notification are the
calculated 1% volume Freestanding Liquids Chart Form (Attached) of procedure CL-CH-PR-252;
Incoming Shipment Inspecting and Sampling, for the following container #’s 5, 12, 41, 42, 43, 45, and 47. The generator was contacted about these unexpected liquids on June 10, 2025. They have begun an
investigation into the characterization of the waste in this shipment. The shipment has been moved
into secondary containment and in “Hold” status pending further investigation and discussions with the generator. The liquids will be managed in accordance with Condition 17.B of Radioactive Material License UT
2300249:

Solid waste received and containing unexpected aqueous free liquid in excess of 1 % by volume shall have the liquid removed and placed in the evaporation ponds or the liquid solidified prior to management.
