and 23; Director Notification and Approval of the LLRW and 11e.(2) CQA/QC Manual
(revision 28c), EnergySolutions hereby provides 48-hour advanced notification of an
equipment change. Attached you will find the equipment documentation, which has been
reviewed and approved by a Utah Licensed Professional Engineer as equivalent.
