Re:No Further Action, Surface Spill, Leaking Petroleum Storage Tank Release
Holiday Oil #68, located at 759 North 1500 West, Orem
Facility ID 1000972, Release Site ONS
Dear Mr. Cahoon:
The file for this release has been reviewed by the Division of Environmental Response and
Remediation (DERR) project manager who has recommended that no further corrective action be
taken at this time. This no further action applies only to the specific Release ONS reported to DERR
on June 15, 2025. This recommendation is based on information contained in the file for the release.
That information was supplied by you, your consultant, or another representative.
The information submitted indicates that any detectable petroleum contamination from this
release is not a threat to human health or the environment according to Utah Admin. Code rule
R311-211. In the future, if other evidence indicates the presence of contamination from this release
which may cause a threat, further corrective action may be required.
If you have any questions, please contact Eileen Brennan, the DERR project manager, at
(801) 536-4100.
Sincerely,
Brent H. Everett, Director

SUBJECT: No Further Action: Surface Spill, Leaking Petroleum Storage Tank Release
Holiday Oil #68, 759 North 1500 West, Orem
Facility ID 1000972, Release ONS
I have reviewed the information for the above-referenced release, which includes information
provided by the facility owner or other representatives. My recommendation that no further action
is required for the release is based on the review of the information contained in the case file, after
due consideration of the corrective action clean-up standards policy (Utah Admin. Code R311-211).
Case file information justifying no further action for the release includes the following:
1. Estimated 4 gallons of unspecified fuel were released to the concrete surface on June
14, 2025, when a customer walked away from their fueling vehicle. Product flowed
a short distance across the concrete surface and entered a dispenser-area catch basin.
2. Absorbents were used on the surface area of the spill by store personnel.
3. Sump and Trap vacuumed out the catch basin.
4. No product reached an oil/water separator or utility. There was no impact to the
subsurface.
No Further Action is recommended for the release. Release data indicate no significant petroleum
impacts exist or remain on-site and do not appear to constitute a current or potential threat to human
health or the environment. This recommendation for close-out is based on the condition that if future
evidence indicates the existence of contamination at or emanating from this site, additional
investigation and corrective action may be required. A No Further Action letter will be sent to the
responsible party.
