This is a Utah Division of Waste Management and Radiation Control (“Division”) staff summary on the inspection of
the 2025 Annual Seeps and Springs Monitoring event (“2025 Event”) that was conducted on June 18, 2025, at the
Energy Fuels Resources (USA), Inc. (“EFRI”) White Mesa Uranium Mill, near Blanding, Utah. The 2025 Event was
inspected against the following enforcement documents:
• The EFRI Quality Assurance Plan (“QAP”) February 15, 2022 (Revision. 7.7)
• Groundwater Discharge Permit No. UGW370004 (“Permit”). Permit version that was signed on March 8,
2021.
• Sampling Plan for Seeps and Springs In the Vicinity of the White Mesa Uranium Mill. Revision 2. July 8,
2016 (“Sampling Plan”).
Summary of Inspection
EFRI informed the Division by email on May 18, 2025 with an attached letter dated May 18, 2025, that EFRI would
be sampling the seeps and springs on June 18, 2025 (”Letter”). The Letter from EFRI provided notice prior to
sampling as required in Section 3 of the Sampling Plan. In Section 2.1 of the Sampling Plan EFRI is required to

sample annually the seeps and springs between May 1 and July 15 each year; therefore, the June 18, 2025, sampling
date meets this requirement.
On June 18. 2025 Division staff (Dean Henderson) observed the EFRI staff (Tanner Holliday and Deen Lyman)
sample Cottonwood Spring, Entrance Spring, and Ruin Spring (see photographs in Figures 1 and 2 in attachment 1
below). The Division staff collected a split sample with EFRI from Cottonwood Spring. The field inspection was
completed with a closeout meeting with EFRI staff Garin Palmer, Tanner Holliday and Deen Lyman. In the meeting
the sampling and inspection activities were reviewed and there were no issues or concerns.
Conclusion
Based on Division staff observation of EFRI staff performing the sampling and monitoring on June 18, 2025, it
appears that EFRI appropriately followed the field requirements in the Permit, QAP and Sampling Plan. The findings
of this inspection and results of the analytical analysis for the Division split sample of Cottonwood Spring will also be
included as part of the review to complete the inspection for the Annual 2025 Seeps and Springs Monitoring Report.
A letter will be drafted notifying EFRI of the closeout of the field inspection and split sampling for the annual 2025
seeps and springs monitoring event.
