Hansen Director M E M O R A N D U M TO: File THROUGH: Adam Wingate Chris Leahy FROM: Dean Henderson DATE: November 19, 2024 SUBJECT: Field Inspection and Split Sampling for the 4th Quarter 2024 Chloroform Groundwater Monitoring Event for Energy Fuels Resources (USA), Inc. White Mesa Uranium Mill, near Blanding, Utah This is a Utah Division of Waste Management and Radiation Control (Division) staff summary on the field inspection and split sampling of the 4th Quarter 2024 chloroform groundwater monitoring event (Monitoring Event) performed on November 5 and 6, 2024, for the Energy Fuels Resources (USA), Inc. (EFRI) White Mesa Uranium Mill, near Blanding, Utah. EFRI is required to submit a written groundwater monitoring report for this monitoring event by March 1, 2025. The monitoring event was inspected against the following enforcement documents: The EFRI Quality Assurance Plan (QAP) dated February 15, 2022 (Revision. 7.7). Summary of Inspection On November 5 and 6, 2024, Division staff Dean Henderson, Adam Wingate and Chris Leahy observed the EFRI staff perform groundwater monitoring activities including the collection of ground water samples from monitor wells for the chloroform groundwater monitoring network. In addition, the Division staff collected a split sample from monitor wells TW4-12, TW4-13, TW4-14, TW4-28, TW4- 31, TW4-36, and TW4-43. A duplicate sample was collected at well TW4-12 and is identified as TW4- 52. See the photographs of sampling activities in figures 1 and 2 in Attachment 1 below. After the inspection and split sampling was completed, a close out meeting was held with the following attendees: Division staff Dean Henderson, Adam Wingate, Chris Leahy and EFRI staff Tanner Holliday, Page 2 Garrin Palmer, and Deen Lyman. In the meeting the Division staff told the EFRI staff that they appeared to follow the QAP. Conclusion Based on Division staff observations of EFRI staff performing the sampling and monitoring on November 5 and 6, 2024, it appears that EFRI appropriately followed the field requirements in the QAP. As stated above, EFRI is required to submit a written report of this Monitoring Event on March 1, 2025. The Division will also review this report and will include split sample analysis for this monitoring event as part of that review to complete the inspection of the 4th Quarter 2024 chloroform groundwater monitoring event.
