The Utah Division of Air Quality (DAQ) received your request on June 6, 2025, for a name and
ownership change for the holder of the above-referenced Small Source Exemption (SSE). According to
your request, we have noted in our records that the holder and party responsible for complying with the
terms and conditions contained in the above-referenced SSE has been changed to ‘Amrize Cement Inc.’.
This change took effect on the date of this letter.

As authorized by the Utah Legislature, the fee for issuing this name change is a one-time filing fee in
addition to the actual time spent by the review engineer and all other staff on the project. Payment should
be sent to DAQ upon receipt of the invoice.

Dear Ms. Tong:
The Utah Department of Environmental Quality, Division of I Air Quality (DAQ), has reviewed your letter
dated October 13, 2003, requesting a small source exemption for a cement distribution terminal and
determined that the small source exemption applies. As a I!esult of this review, the Holcim (U.S.) Inc.
cement distribution terminal will be granted a Small Sourc~ Exemption,and the current Approval Order
(DAQE-321-99) will be rescinded.
Holcim (U.s.) Inc. shall remain subject to the limitations and conditions contained in the Small Source
Exemption (DAQE-EN2074003-O3), dated November 19, 2003. If you have any questions please contact

For the Devils Slide and L Site, the current name should be Holcim (US) Inc. The new name will be Amrize Cement Inc.

Hi Derrick,
I have just been assigned the name changes we spoke about earlier this morning for the three (3) Holcim sites. I am
taking a second look to see if I can find the Small Source Exemption DAQE-GN2074003-03. If you have this file on hand,
could you send it to me?
Attached are the permits for the two (2) other sites we discussed: DAQE-AN100070031-22 (Devils Slide) and DAQE-
AN106760028-22 (Utelite/Rockport). Please review these permits, especially the Utelite one, and make sure they line up
with the permits you are requesting a name change for.

Thank you for attaching the SSE Letter. The name change application has been received by the DAQ and you may
submit a prepayment or wait to be invoiced.
A "Holcim Solutions and Product US, LLC" has a permit for a Foam Manufacturing Facility located in Salt Lake City. Is this
the same company and would you like this to be included in the name change? I have attached the permit.

Thanks for giving me a heads up on the progress. We will be deciding on the Foam Manufacturing Facility
and submitting the payment soon.

Just to clarify, we will also be able to make the name change for the Lehi Small Source Exemption DAQE-
