Terracon developed an Analysis of Brownfield Cleanup Alternatives [(ABCA); dated March 15, 2021] for the site. The Recommended Cleanup Alternative selected as most efficient, cost effective, and reliable was Maintenance of Hardscape and a Sub-slab Depressurization System (SSDS).Based on the selected cleanup strategy, a Remedial Action Plan (RAP; Terracon, June 2, 2021) was developed for the site and approved by the DERR on July 27, 2021. The purpose of this Site Management Plan (SMP) is to maintain the protectiveness of the remedial action. This SMP applies specifically to Parcel A, which is addressed as 155 South 600 West, Salt Lake City, Utah, and has also been referred to as the “southern” parcel in previous documents for the CCM VCP site. Parcel A is explicitly defined in the Alta Survey provided in Appendix B. An Environmental Covenant (EC) and this SMP will be incorporated into the final Certificate of Completion issued by the UDEQ. This SMP discusses specific details and procedures related to the Activity and Use Limitations and Engineering and Institutional Controls imposed upon the Property through the recorded EC. The SMP will be implemented by CCM. The SMP may be amended from time to time with the acceptance of the UDEQ. Site access and reimbursement of UDEQ oversight costs under this SMP are addressed in the EC. REMEDIAL ACTION To protect human health, the selected remedy was to maintain the current asphalt covering of the site to prevent contact with impacted soils, and to install a vapor mitigation system (VMS) consisting of a sub-slab depressurization system (SSDS) within the building, designed to remove contaminant vapors from beneath the building footprint and prevent vapor intrusion of the contaminants. The SSDS was installed in October of 2021. A copy of the SSDS Design and Specifications is included in Appendix C. SITE MANAGEMENT At this time, redevelopment of Parcel A does not include excavation of soil or soil disturbance, or demolition of the current structure. Site Management Plan – Parcel A Centro Civico Mexicano (VCP Site #C101)■ Parcel No. 15-01-108-036 March 29, 2022 ■Terracon Project No. AL157355 Responsive ■Resourceful ■Reliable 3 3.1 Environmental Covenant CCM has prepared an Environmental Covenant that outlines the continuing obligations and limitations associated with impacted soils remaining on the site and the maintenance and operation of the SSDS. The Environmental Covenant will be recorded on the Site’s title. 3.2 Institutional Controls The following land use limitations are imposed upon the Site: n Land use will remain commercial (no residential land use) at and below ground level. n Uses that include managed care facilities, hospitals or any type of business that would require a caretaker to reside on the facility are not approved as future uses on the ground floor or below. n Commercial uses that would expose children to hazardous constituents at the site for extended periods of time (such as day care, school, and playground/play area facilities) are also not approved at ground level or below. n Residential land use is prohibited below grade or at the ground level of the Property. n Groundwater will not be accessed for drinking water, irrigation, or bathing purposes. CCM will certify that land use and development restrictions are being followed on an annual basis. The Annual Land Use Certification Form to be submitted to UDEQ is included as Appendix D. 3.3 Engineering Controls The following engineering controls are applied to the site to prevent exposure to environmental impacts: n A vapor intrusion mitigation system has been installed at the site in the form of a sub- slab depressurization system (SSDS). Operation of the system will be documented quarterly by a visual inspection of the exterior gauges and the alarm system. n The remedy for exposure to impacted soils will be to maintain a hardscape surface over the Site to prevent contact with the soils. This will include a hardscape surface completion and building footprint. The asphalt surface completion covering the site must remain in a condition such that soils at the site are not exposed. The condition of the asphalt surface will be inspected on a quarterly basis. The Owner will inform any contractors performing maintenance or building renovation/demolition of the presence of the SSDS prior to initiating site work. If repair or maintenance of the current asphalt hardscape is necessary, CCM will submit notice to UDEQ and provide detail as the Site Management Plan – Parcel A Centro Civico Mexicano (VCP Site #C101)■ Parcel No. 15-01-108-036 March 29, 2022 ■Terracon Project No. AL157355 Responsive ■Resourceful ■Reliable 4 specification of the repair. If hardscape removal or soil disturbance is required, the guidelines detailed in Section 3.4 will be followed. CCM will certify that the engineering controls installed at the site are intact and operational on a quarterly basis. The Quarterly Engineering Controls Inspection Form to be submitted to UDEQ is included as Appendix D. CCM may choose to reevaluate soil gas conditions in the future after the system has operated for at least one year and then petition to discontinue operation of the SSDS system which is an option if soil gas concentrations fall and stay below their respective vapor intrusion screening levels (VISLs). If CCM chooses to petition to discontinue operating the SSDS system, CCM will prepare a work plan proposing sampling to evaluate soil gas conditions and potential rebound of contamination for UDEQ review and approval. 3.4 Soil Disturbance Requirements As COCs remain in soils at the Site above unrestricted land use regulatory screening levels, any excavation of soils, building demolition or redesign, or change in land use anywhere on the Site will require that a Work Plan that includes a Quality Assurance Project Plan (QAPP) be submitted to UDEQ for review and approval prior to implementing work at the Site. Construction activities and other unforeseen events or disturbances that may result in disturbances to impacted soil will require CCM to submit a work plan for approval by the UDEQ to assess risks to human health and the environment and to properly manage the excavated materials. The mitigation plan shall include construction worker notification and safety measures and appropriate soil waste characterization and disposal requirements, if required. Impacted soils to be transported off-site may require characterization by sampling and laboratory analysis prior to approval for off-site disposal. Characterization of the soils will determine the proper disposal methods. In all cases, soil must be disposed of in a lawful manner consistent with all pertinent federal, state, and local environmental laws, and documentation must be provided to the UDEQ. In the event that sampling activities are required to be conducted, all sampling must be performed by qualified personnel. Samples must be analyzed by state-certified laboratories and quality assurance procedures in the Centro Civico Mexicano Quality Assurance Project Plan will be followed as appropriate. 3.5 Worker Health and Safety Requirements The Owner will inform the UDEQ of any excavation of soils or sub-grade building renovation/demolition prior to initiating site work. Contractors that could come into contact with soil impacts will prepare a Health and Safety Plan that specifically addresses the tasks and potential contaminants that could be encountered and any appropriate procedures and personal protective equipment (PPE) that are required. It will be the Contractor’s responsibility to ensure worker protection and training meet the Occupational Safety and Health Administration’s (OSHA’s) requirements. 3.6 Contingency Plan If unforeseen events or unexpected contamination are encountered, UDEQ will be notified and a contingency plan will be developed in consultation with UDEQ to address the situation. 3.7 Notification CCM shall notify UDEQ either by phone at (801) 536-4100 or in writing at the address noted below of any excavation into impacted soils. In addition to the SMP requirements, site activities must comply with appropriate permitting authorities. System operation checks were installed as a way to validate the SSDS is operating in the form of three (3) audible alarms inside the building and 3 external pressure gauges at each of the 3 blowers (see photolog). As required by the Site Management Plan (SMP) for Parcel A (“Property”) of the Centro Civico Mexicano VCP site, I hereby certify that I have observed the following two system operation checks: A.Audible alarms were installed at 3 vapor extraction points of the system. The audible alarms (please select one): [ ] have not sounded. ; or [ ] have sounded and been reported to the appropriate party and repaired. ; or [ ] have sounded and have not been reported/not been repaired. B.I have visually inspected the external pressure gauge at each of the three (3) blowers. Each pressure gauge (please select one): [ ] showed a vacuum of a minimum 1-inch w.c. ; or [ ] one of more of the gauges showed a vacuum less than 1-inch. It was reported to the appropriate party and the system has been adjusted to increase the vacuum in that vapor extraction point (VEP). ; or [ ] one of more of the gauges was below 1-inch w.c.
