This letter serves as notification that the routine source chemical monitoring requirements for
Rollins Well #1 (WS001) and Rollins Well #2 (WS002) have been established. Please ensure that
the samples listed below are collected upstream of all service connections as water exits from the
Mountain Green Mutual Water Co Tank (ST001) by the specified due dates. All samples collected
from this location should be labeled as collected at Sampling Station -01 02 (SS001).

Additionally, please ensure that “SS001” is written in both the facility and sampling point ID
columns on the laboratory chain of custody form for all samples from this location.

Sampling Station – 01 02 represents the combined flows of the Rollins Well #1 (WS001) and the
Rollins Well #2 (WS002) as it exits the Mountain Green Mutual Water Co Tank (ST001). Please
collect the samples below when both wells are represented in the tank.

Below are the current monitoring requirements for Sampling Station -01 02 (SS001). An updated
monitoring schedule can be viewed any time at waterlink.utah.gov under the water monitoring
section. Please contact David Kruse at [phone] or [e-mail] with any questions
regarding the chemical monitoring requirements for this system.

PFAS Information and Resources
The EPA rule for PFAS requires that either method 533 or 537.1 version 2 are used for testing.
Please ensure that the chosen laboratory processes PFAS samples using one of these methods.
Additionally, the Division has resources to assist water systems in completing the PFAS initial
monitoring requirements. For more information on the resources the division can provide related to
