Dear Mr. Gilgen,
The New Source Performance Standards for Small Industrial-Commercial-institutional Steam
Generating Units (NSPS Boilers) 40 CFR 60 Subpart Dc reporting the quantity of fuel other than
natural gas combusted during the reporting period for primary The Orthopedic Specialty Hospital
(TOSH) Utah DAQ Site ID 10405.
-Six month reporting period covered under this is from July 1, through December 31, 2023.
-Boiler hours of operation on diesel fuel for the reporting period is as follows:
o Boiler #4 – 3 hours
-There were no fuel deliveries during the reporting period. A copy of the diesel certification
from the most recent fuel delivery is included as reference.
In accordance with Utah Administrative Code R307-415-5d and based on information and belief
formed after reasonable inquiry, I certify that the statements and information in this document are
true, accurate, and complete.
