The Boil Order needs to be delivered to the customers by 1pm Saturday June 28, 2025. Please reply all to this email chain with a boil order prepared using the attached template. Attached are the four documents needed for completing a boil order. The Boil Order needs to be filled out and sent back to me for approval. Once it is approved you can move forward with delivering it to the customers. Boil Order Example: This is going to help you to write your own boil order. Feel free to work on this document or the Boil Order PN if you would like. Boil Order PN: This is the template we have for systems to fill out their information and send it back for approval. Division of Drinking Water Boil Order Instructions: This will provide you with instructions on many things boil order related (including ways to deliver notice). Certification for Boil Order: this document is the certification form that needs to be filled out after you deliver the boil order to the customers. Please send us the boil order template back for approval as soon as possible. We want to make sure it gives you all time to deliver it to the customers before the 24 hour deadline tomorrow. I will keep an eye on it throughout today. If I do not see it by tonight, Tom McOmie (tmcomie@utah.gov or (385) 552-4446) will check back again tomorrow morning.
