RE: Operating Permit Request for Mayflower/Lincoln Booster Station #2 (PF019)

This letter’s purpose is to inform the Division of Drinking Water that JSSD’s Mayflower/Lincoln Booster Station #2 has been constructed, pressure tested, and disinfected. JSSD has requested that we provide adequate information to your office so that an Operating Permit may be obtained. As the newly appointed District Engineer, I have visited the site and taken the attached pictures.

The following details each item of the Operating Permit Checklist (attached):
• Certification of Rule Conformance: Provided on the attached letter by the PE of Record Nick Graue, PE.

• Utah Registered Engineer’s Statement: Provided on the attached letter by the PE of Record Nick Graue, PE.
• JSSD has received the As-Built Drawings for the pump station.
• Clayton Riding, JSSD Inspector, was present during the disinfection and pressure testing of the pump station and associated water lines. He witnessed that the water
line was properly flushed and disinfected per AWWA C651 & 654.
• Water quality sampling data is attached for the pump station.

As the Utah-licensed professional engineer of record for the above-referenced public drinking
water construction project, I hereby certify that, to the best of my knowledge and belief:

1. The project was completed in accordance with the approved engineering plans and
specifications, and
2. All applicable requirements of Utah Administrative Code R309-500 through R309-550
have been satisfied, including but not limited to design, construction, and material
standards, and
3. Any changes made during construction were reviewed and determined to be in
compliance with the applicable rules and do not compromise public health protection or
system performance.

This certification is based on my involvement in the design and construction observation of the
project, including review of construction records and coordination with the owner and contractor.
