Subject: Notice of Decreased Monitoring for Oak Haven Water Company, UTAH26043 Robert Allen:
This letter serves as notification that quarterly monitoring for radionuclides is no longer required from the Arsenic Treatment Facility (identified as TP003). A radionuclide sample collected from the Arsenic Treatment Facility (TP003) is now required once every nine years. The next radionuclide sample is due sometime between April 1, 2029 and June 30, 2037.
Following the issuance of an operating permit the Arsenic Treatment Facility (TP003) was placed on a quarterly monitoring requirement to fulfill the initial monitoring requirements given in R309-205-7(1)(b). A sufficient number of radionuclide samples have been collected to qualify this source for reduced monitoring.
The updated source chemical monitoring requirements for the Arsenic Treatment Facility (TP003) are given below and are available to view anytime on waterlink.utah.gov. Please contact David Kruse at dbkruse@utah.gov or 385-566-7789 for questions regarding the source chemical monitoring requirements for this system.

PFAS Information and Resources The EPA rule for PFAS requires that either method 533 or 537.1 version 2 are used for testing. Please ensure that the chosen laboratory processes PFAS samples using one of these methods. Additionally, the Division has resources to assist water systems in completing the PFAS initial
monitoring requirements. For more information on the resources the division can provide related to PFAS initial monitoring please visit ddwpfas.utah.gov. For PFAS related questions, please contact John Steffan at jtsteffan@utah.gov or (385) 499-3926. Sincerely,
