the Division of Water Quality (DWQ) for modification of the UPDES permit referenced above. The permitted Outfall 007 location has been updated as requested in your letter dated April
25, 2025. Since the time of your request, DWQ has tracked the permit writer’s time spent to modify the permit as appropriate. The UPDES Permit as modified will be sent under separate
cover in the coming weeks. A fee schedule was included in the Utah Department of Environmental Quality Budget appropriation request at the direction of the Legislature and in accordance
with Utah Code annotated 19-1-201. The fee schedule, as approved by the legislature, includes a prescribed fee for specific Industrial Categories and activities. The prescribed fee
for this permit activity is $125.00 per hour and a total of four(4) hours was spent on the modification of the UPDES Permit referenced above (see attached invoice). Therefore, please
remit $500.00 within 30 days of receipt of this letter to:

Payment may also be via the Payment Portal at https://deq.utah.gov/payment-portal using the Water Quality Shopping Cart and selecting Invoice Payment. Please be sure to include the invoice
number with either method of remittance.
