RE: Operating Permit Request for Hole-in-the-Rock Well #2 (WS008); Oquirrh Mountain Water
Company, System #23044, File #15783
Mr. Arns:
The purpose of this letter is to request an Operating Permit for the Hole-in-the-Rock Well #2 (HITR
2) Well House in Lake Point, Utah on behalf of Oquirrh Mountain Water Company (OMWC). The
well house and yard piping have been constructed. The well house piping and associated yard
piping have been leak tested and disinfected.
The following items detail compliance with the Operating Permit Checklist:
1. Certification of Rule Conformance:
o As a registered professional Engineer in the State of Utah, I confirm that all
conditions of the Plan Approval were accomplished.
o As a registered professional Engineer in the State of Utah, I certify that the project
(including changes to the project), to the best of my knowledge, have been built in
accordance with Utah Administrative Code.
2. As-built or Record Drawings: While there have been minor revisions reflected in the as-
built redlines provided by the contractor, they are substantially the same as the approved
pre-construction drawings previously submitted to the State. Some examples of the minor
revisions are:
o Client requested removal of trail system and stairs from project.
o Simplification of bends and fittings during construction that were not needed.
o Slight rerouting of the roof gutter downspouts.
Record drawings are actively being prepared to reflect these minor revisions, but they do
not functionally impact the operation of the well house. The approved pre-construction
drawings meet the requirements to comply with the checklist.
3. Confirmation of water system receipt: OMWC retained a copy of the approved pre-
construction drawings.
4. Bacteriological samples:
o Standards for Disinfecting Water Mains: OMWC personnel observed the
disinfection and flushing of the piping to ensure accordance with ANSI/AWWA
C651-14. The samples were collected from both ends of the newly installed
pipeline. Please see the attached bac-T sample results.

o Standard for Disinfection of Water-Storage Facilities: N/A
o Standard for Disinfection of Water Treatment Plants: N/A
o Standard for Disinfection of Wells: Two consecutive samples, taken greater than
30 minutes apart, are attached. Both bacteriological samples were negative,
indicating that the well has been properly flushed and disinfected.
5. Water quality data: New source water quality data was previously provided during the plan
review process.
6. Plan review requirements: N/A
7. O&M manuals: O&M Manuals have been submitted by the contractor to OMWC.
8. Location: The new source location was provided during the pre-construction approval
process.
