(EFRI) provided the Division Waste Management and Radiation Control (Division) written Notice concerning the pumping failure at pumping well TW4-19 (DRC-2025-001849). The Notice explained how EFRI staff discovered silica sand filler pack material in the pump(s), flow meter, and discharge lines associated with TW4-19. Well logs indicate the well was completed at a depth of 125 feet below ground surface (bgs). Upon further inspection, EFRI staff found that the bottom of the well was now 115 feet bgs, indicating the bottom 10 feet of the well was filled with silicate sand. This would seem to indicate that the well screen collapsed between 115 and 125 feet bgs and that TW4-19 can no longer be used as a pumping well. The Division agrees with this assessment. Because TW4-19 is no longer serviceable, the Notice requested toabandon TW4-19 and install a new pumping well identified as TW4-19A. Again, the Division concurs and approves the abandonmentof TW4-19 and installation of a new pumping well. Before beginning the abandonment of TW4-19 and installation of TW4-19A, please submita plan for Division approval containing the following: Well abandonment in accordance with Utah Administrative Rule R655-4-14 New well location All applicable requirements from the Groundwater Corrective Action Plan for Chloroform Part II.C and H.
