RE: Operating Permit Request for Cottam Well 3

Dear Paul;

The Washington County Water Conservancy District (WCWCD) has requested that we submit
this operating permit request letter to you to for the issuance of an operating permit for File
#12871 – Cottam Well #3 and Cottam Well #3 Chlorinator (WS006 and TP004). In accordance
with R309-500-9 we certify the following:

1) No significant changes were made to the plans for Cottam Well 3.
2) Conformance with the plan approval conditions.
3) O&M Manuals were given to the owner.
4) As-built drawings are provided to the Owner and DEQ with this letter.
5) Proper flushing and disinfection were accomplished according to ANSI/AWWA C654-97
standards and Bac-T results are included with this letter.
6) Water quality data was provided during the equipping approval.

The District would like to introduce the well into the Cottam System as soon as possible.
