waste inspection that was conducted at our facility on September

Action Taken: Mountain States Analytical Inc. t-clip tested filters on 2l ll99 results will be
back in15 working days and forwarded to your office.

Action Taken: Have had laminated cards made (see attached) to post by each company
phone.

Action Taken: Have serrt letter (see attached) to local police, fire and hospital for emergency
