Follow Up to Notification of Discrepancy with 9075-01-0074
Dear Mr. Hansen, In a letter dated May 20, 2025 (CD-2025-108), EnergySolutions notified the Director of the Utah
Division of Waste Management and Radiation Control (DWMRC) of a discrepancy with Shipment
9075-01-0074 (Bates Number M16765) (GSAP # 0209001601). Follow up to that notification is provided herein. Condition Report # CR-2025-0536 has been written to track resolution of this issue. Shipment 9075-
01-0074 arrived at the Clive Facility on May 19, 2025. The shipment arrived on a flatbed trailer (#
OTR117) containing 16 total containers (9 metal boxes, 3 totes, and 4 drums) As mentioned previously, during Clive’s routine acceptance survey CL-RS-PR-115 F1 (see attached), dose rates remained well below 49 CFR 173.441(a) requirements but were in exceedance at 11 mrem/hour at 2 meters (in excess of allowable limits of the Waste Acceptance Criteria of Radioactive Materials
License UT 2300249 and 49 CFR 173.441(b)(3)).
The generator was contacted the same day on May 19, 2025. The shipment has been moved into the Restricted Area and in “Hold” status pending further investigation and discussions with the generator. The containers are currently being stored in compliance with CL-RS-PR-150; Posting
Requirements for Radiological Hazards, within a Radiation Area on the Mixed Waste Storage Pad
B. Please consider this the follow-up written notification (required in Radioactive Materials License UT2300249, Condition 68) for this discrepancy.
