Subject: Radioactive Material License UT 2300249; 24-Hour Notification of Potential Discrepancies with Shipment 9075-01-0074 Dear Mr. Hansen,

EnergySolutions herein provides the Director of the Utah Division of Waste Management and Radiation Control with 24-hour notification of potential discrepancies with Shipment 9075-01-0074 that arrived at the Clive Facility on May 19, 2025. The shipment was comprised of 16 B-25 boxes, 10 metal boxes, 3 flex bins, 5 metal drums, and a poly drum of debris for macroencapsulation
treatment. During Clive’s routine acceptance surveys, 60 mr/hr was observed on the plane and 11
mr/hour at 2 meters (in excess of allowable limits of the Waste Acceptance Criteria of Radioactive Material License UT2300249 and 49 CFR 173). Further receipt processing of the shipment has been halted and the generator notified of the potential discrepancies.
A Condition Report is being generated to track resolution of the discrepancy. The Clive Facility is
coordinating with the generator for proper management of this shipment. Please consider this the required 24-hour notice. A 7-day follow-up notification with further details will be provided to the Director by May 26 2025.
