Follow Up to Notification of Discrepancy with 9125-13-0007
Dear Mr. Hansen,
In a letter dated April 30, 2025 (CD-2025-084), EnergySolutions notified the Director of the Utah
Division of Waste Management and Radiation Control (DWMRC) of a discrepancy with Shipment
9125-13-0007 (Bates Number M16438) (GSAP 1307008022). Follow up to that notification is
provided herein.
Condition Report # CR-2025-0457 has been written to track resolution of this issue. Shipment 9125-
13-0007 arrived at the Clive Facility on August 16, 2024. As mentioned previously, in accordance
with the Waste Characterization Plan of the Radioactive Materials License UT 2300249 (RML),
sampling for radiological parameters was not required for this shipment based on frequency
requirements. On April 29, 2025, while preparing the shipment for Thermal Desorption Treatment,
sampling was conducted for nitrate and radiological parameters in accordance with the RML and
Clive Work Instruction CL-LB-WI-024. A homogenous sample was taken and analyzed using
Onsite gamma spectroscopy. The results (see attached) for Am-241 reported at 490,000 pCi/g (limit-
10,000 pCi/g) exceeding Class A waste limits.
Condition 15. A of the RML requires:
Prior to management of any low level radioactive or mixed wastes requiring treatment
before disposal, the Licensee shall, based on knowledge of the technology to be used for
treatment/processing of each particular radioactive or mixed waste, calculate and document
that the radionuclide concentrations of the resultant processed waste do not exceed Class A
waste limits.
As a result, a sample has been sent off-site for third-party analysis to confirm findings. Results have
not been received and will be provided to the Director upon receipt.
The generator was contacted the same day on April 29, 2025, and a meeting to discuss the
discrepancy with the generator was held on April 30, 2025. The shipment has been placed in Hold
status pending further investigation and results from the third-party analysis. Additionally, as part of

corrective actions for this discrepancy, all profiles from this generator have been suspended until a
formal Corrective Action Plan is submitted and approved.
Please consider this the follow-up written notification (required in Radioactive Materials License
UT2300249, Condition 68) for this discrepancy.
