Shipment 9125-13-0007 (Bates Number M16438) (GSAP 1307008022) arrived on August 16, 2024. The Shipment was accepted, and a deferred chemical sample was collected on August 19, 2024, in accordance with Attachment II-1, Waste Analysis Plan of the state-issued Part B Permit. Per the Waste Characterization Plan of the RML, sampling for radiological parameters was not required for this shipment based on frequency requirements. On April 29, 2025, while preparing the shipment for Thermal Desorption Treatment, additional sampling was conducted for nitrate and radiological parameters in accordance with Clive Work Instruction CL-LB-WI-024. Onsite gamma spectroscopy results did not align with the manifested data. As a result, a sample has been sent off-site for third-party analysis to confirm findings. The shipment has been placed in Hold status pending further investigation. A Condition Report is being generated to track resolution of the discrepancy. Please consider this the required 24-hour notice. A 7-day follow-up notification with further details will be provided to the Director by May 7, 2025. Should there be any questions with this notification, please contact myself or Vern Rogers at 801-649-2000. Sincerely, Steve D. Gurr Environmental Engineer and Manager Digitally signed by Steve D.
