INTRODUCTION On March 27 and 28, 2025, Atlas Technical Consultant (Atlas) conducted a groundwater monitoring event at the former Western Transmission Facility located at 150 North Carbonville Road, Price, Utah (see Figure 1, Site Map). The work was performed on behalf of the Utah Department Environmental Quality, Division of Environmental Response and Remediation (DERR) under Work Assignment HZZ-33-A1. The approved work plan included gauging of all the monitoring wells and sampling wells MW-3, MW -6, MW -7, MW -8, MW -13, MW -15, MW -17, MW -18, and MW -20 through MW-24. GROUNDWATER MONITORING FIELD PROCEDURES The field and laboratory procedures utilized to collect the information presented in this report are discussed in the following sections. Field Parameters, Well Purging Procedures, and Chemical Analysis The approved work plan included the collection of groundwater elevation data, sampling the monitoring wells, and preparing a summary report. Free product was not measured in any of the wells during the March 2025 sampling event. A site map depicting the monitoring well locations and surrounding area features is presented as Figure 1. Prior to well purging, groundwater depths were measured in each well using an electronic water level/free product interface probe. The water level/free product interface probe was decontaminated between each well with an Alconox detergent wash and tap water rinse. 358 South 700 East, B518 Salt Lake City, Utah 84102 Office: 801.935.4917 www.oneatlas.com Page 2 ATLAS Following field parameter measurements, the monitoring wells to be sampled were purged a minimum of three well volumes or until dry using new, factory-sealed, disposable PVC bailers. Following the purging process, the wells were allowed to recharge to within 95 percent of the pre-purge water level and then sampled. The samples were transferred to appropriate containers and immediately placed on ice in an insulated cooler and then transported under chain-of-custody documentation to a Utah-certified laboratory for analysis. The groundwater samples were analyzed for methyl t-butyl ether, benzene, toluene, ethyl benzene, total xylenes, naphthalene (MBTEXN) using EPA Method 8260; and total petroleum hydrocarbons as gasoline (TPH-GRO), and TPH as diesel (TPH-DRO) using EPA Method 8015. A field blank sample was also collected. The laboratory report and chain of custody are presented in Appendix A. No free product absorbent socks were placed in any of the monitoring wells in December 2024, so there were no socks to remove during the March 2025 sampling event. Monitoring well MW-9 was destroyed/paved over during the spring of 2023. Historically, free product had been measured in MW -9. Given that free product was not measured in any of the wells during the March 2025 event, no product absorbent socks were placed in any of the monitoring wells. Field Parameter and Chemical Analysis Results Field parameter measurements and groundwater laboratory results are presented in Table 1. The March 2025 groundwater elevation data were utilized to prepare a groundwater contour map (Figure 2) for the site. The data indicate that the groundwater gradient appears to be toward the west, with a gradient of approximately 0.0306 ft/ft measured between MW -18 and MW -20. The March 2025 laboratory results for the samples collected from on-site monitoring wells (MW - 3, MW -6, MW -13, MW -15, and MW-20) indicated no detection of MBTEXN, TPH-GRO, or TPH- DRO above their respective Tier 1 Screening Levels. The March 2025 laboratory results for the samples collected from the off-site monitoring wells indicate the presence of benzene above its Initial Screening Level (ISL) of 0.005 mg/L in MW -8 (1.580 mg/L). In addition, ethylbenzene was detected above its ISL of 0.7 mg/L in MW -8 (1.150 mg/L). No other MBTEXN constituents were measured above their respective ISLs in any of the other off-site wells sampled. The benzene isoconcentration map is presented as Figure 3. The TPH-GRO ISL of 1.0 mg/L was exceeded in MW -8 (9.300 mg/L). TPH-GRO was not measured above the ISL in any of the other off-site wells sampled. The TPH-GRO isoconcentration map is presented as Figure 4. The TPH-DRO ISL of 1.0 mg/L was exceeded in MW -7 (2.550 mg/L), MW -8 (5.800 mg/L), MW -21 (5.0900 mg/L), and MW -23 (1.690 mg/L). TPH-DRO was not measured above the ISL in any of the other off-site wells sampled. The TPH- DRO isoconcentration map is presented as Figure 5. To aid in long-term data trends, hydrographs were prepared for MW -8 and MW -15. These are presented as Figures 6 and 7; and are discussed in the following paragraphs. These wells were selected based on location in relation to the release source and historically impacted areas. The hydrograph for MW -8 (Figure 6) indicates that the groundwater elevation has increased since the December 2024 monitoring event. Historical data indicates that benzene concentrations have fluctuated between 0.983 mg/L (June 2020) to 3.93 mg/L (May 2011). In March 2025 the benzene concentration was measured at 1.580 mg/L. Historical data indicates that the TPH- GRO concentrations have fluctuated between 2.7 mg/L (June 2008) to 115.00 mg/L (September 2012). In March 2025 the TPH-GRO concentration was measured at 9.300 mg/L. Page 3 ATLAS The hydrograph for MW -15 (Figure 7) indicates that the groundwater elevation has decreased since the December 2024 monitoring event. The benzene concentrations have fluctuated between 0.00601 mg/L (March 2025) to 0.420 (May 2012). The data collected in 2020 indicated a steady decrease; however, since 2021, the benzene concentrations in MW -15 have increased or decreased in correlation to decreasing or increasing water levels. The benzene concentration for March 2025 was 0.00601 mg/L, which is the lowest benzene concentration since the well was first monitored on January 11, 2011. TPH-GRO concentrations have decreased since the December 2024 monitoring event. The TPH-GRO concentration for March 2025 was 0.735 mg/L. CONCLUSIONS AND RECOMMENDATIONS Free product was not measured in any of the wells during the March 2025 sampling event. Laboratory results for on-site wells indicated no detection of MBTEXN constituents above their respective Tier 1 Screening Levels. The TPH-GRO and TPH-DRO Tier 1 Screening Levels were not exceeded in any of the on-site monitoring wells sampled. Laboratory results indicate that the benzene ISL was exceeded in off-site well MW -8. In addition, the ethylbenzene ISL was exceeded in MW -8. TPH-GRO was measured above the ISL of 1.0 mg/L in MW -8. TPH-DRO exceeded the ISL of 1.0 mg/L in MW -7, MW -8, MW -21, and MW -23. No other ISLs were exceeded in any of the other off-site monitoring wells sampled. In June of 2010 approximately 1,461 yd3 of impacted soil were removed from the site and transported to the Emery County Landfill for disposal. The results of the confirmatory soil sampling suggested that significant adsorbed phase hydrocarbons were successfully removed during the excavation activities; however, the data from the March 2025 monitoring event indicate that groundwater impacts above ISLs are still present off-site. An expanded subsurface investigation was performed in December 2023. Five monitoring wells (MW -20 through MW-24) were installed in the general vicinity of the former Aboveground Storage Tank (AST) facility and to the southwest off site. An attempt was made to obtain an access agreement from the automobile dealership located to the west beyond Carbonville Road to install additional wells; however, the property owner denied access to the property, limiting the attempt to define the extent of free product impacts. The March 2025 groundwater monitoring event concluded the monitoring activities scheduled under work assignment HZZ-33-A1. Based upon the March 2025 laboratory results and historical groundwater monitoring data collected at the site, Atlas recommends that the groundwater monitoring program continue at the site. Results relate to the accuracy of this information provided, and as the samples are received. Abbreviations and Definitions MDL Method Detection Limit. RDL Reported Detection Limit. Rec.Recovery. RPD Relative Percent Difference. SDG Sample Delivery Group. (S) Surrogate (Surrogate Standard) - Analytes added to every blank, sample, Laboratory Control Sample/Duplicate and Matrix Spike/Duplicate; used to evaluate analytical efficiency by measuring recovery. Surrogates are not expected to be detected in all environmental media. U Not detected at the Reporting Limit (or MDL where applicable). Analyte The name of the particular compound or analysis performed. Some Analyses and Methods will have multiple analytes reported. Dilution If the sample matrix contains an interfering material, the sample preparation volume or weight values differ from the standard, or if concentrations of analytes in the sample are higher than the highest limit of concentration that the laboratory can accurately report, the sample may be diluted for analysis. If a value different than 1 is used in this field, the result reported has already been corrected for this factor. Limits These are the target % recovery ranges or % difference value that the laboratory has historically determined as normal for the method and analyte being reported. Successful QC Sample analysis will target all analytes recovered or duplicated within these ranges. Original Sample The non-spiked sample in the prep batch used to determine the Relative Percent Difference (RPD) from a quality control sample. The Original Sample may not be included within the reported SDG. Qualifier This column provides a letter and/or number designation that corresponds to additional information concerning the result reported. If a Qualifier is present, a definition per Qualifier is provided within the Glossary and Definitions page and potentially a discussion of possible implications of the Qualifier in the Case Narrative if applicable. Result The actual analytical final result (corrected for any sample specific characteristics) reported for your sample. If there was no measurable result returned for a specific analyte, the result in this column may state “ND” (Not Detected) or “BDL” (Below Detectable Levels). The information in the results column should always be accompanied by either an MDL (Method Detection Limit) or RDL (Reporting Detection Limit) that defines the lowest value that the laboratory could detect or report for this analyte. Uncertainty (Radiochemistry)Confidence level of 2 sigma. Case Narrative (Cn) A brief discussion about the included sample results, including a discussion of any non-conformances to protocol observed either at sample receipt by the laboratory from the field or during the analytical process. Quality Control Summary (Qc) This section of the report includes the results of the laboratory quality control analyses required by procedure or analytical methods to assist in evaluating the validity of the results reported for your samples. These analyses are not being performed on your samples typically, but on laboratory generated material. Sample Chain of Custody (Sc) This is the document created in the field when your samples were initially collected. This is used to verify the time and date of collection, the person collecting the samples, and the analyses that the laboratory is requested to perform. This chain of custody also documents all persons (excluding commercial shippers) that have had control or possession of the samples from the time of collection until delivery to the laboratory for analysis. Sample Results (Sr) This section of your report will provide the results of all testing performed on your samples. These results are provided by sample ID and are separated by the analyses performed on each sample. The header line of each analysis section for each sample will provide the name and method number for the analysis reported. Sample Summary (Ss)This section of the Analytical Report defines the specific analyses performed for each sample ID, including the dates and times of preparation and/or analysis.
