This email is to notify the Division of Drinking Water and the Davis County Health Department that West Bountiful City Water System
UTAH06014 – has fully disconnected all Fluoridation Equipment from our water wells and water distribution system. All Fluoride
injection equipment has been removed, dosing pumps turned off and capped, valves on chemical storage tanks are turned in the off
position. Our City has been given quotes on the removal of the liquid fluoride from a certified hazardous waste disposal company and is
in process of scheduling them to remove the chemical and clean our storage tanks. When this is completed, pictures of the empty and
cleaned storage tanks will be sent to the division of drinking water / Environmental Quality (DEQ).

West Bountiful City stopped pumping Fluoride on April 7, 2025, and removed the fluoride injection equipment from our
water distribution system on April 8, 2025. The West Bountiful City Water System’s Finial Fluoride Use Report for April
2025 was submitted to the Davis County Health Department and the Division of Drinking Water. On June 4, 2025 –Blake Anderson with West Bountiful City Water Department, Water System UTAH06014 met with Angie Jone of the
Davis County Health Department to visit each well site and fluoride room to verify that all fluoride equipment has been
fully disconnected from the water distribution system. Please see attached pictures.
