Dear Mr. Marvin:
On May 31, 1991, the Division of Environmental Response and Remediation (DERR)
received a petroleum release report for this site. Following consultation with your Certified
Consultant, the DERR project manager recommends performing two rounds of groundwater
monitoring and sampling of wells MW-1 and MW-A. If either or both of the wells are no longer
deep enough for valid groundwater sampling, the wells shall be deepened or replaced, followed by
the necessary groundwater monitoring events.
All environmental samples must be collected by a Utah-Certified sampler following
sampling protocols as outlined in Utah Admin. Code R311-205. The report associated with the first
sampling event (and/or well replacement) must be submitted to our office within 90 days from the
receipt of this letter. The report for the second event must be submitted within 30 days of the
sampling event. Environmental samples must be analyzed for gasoline constituents using the
methods indicated in the Environmental Media Sampling Handbook (June 1, 2021) available on the
DERR website.
Following completion of the two rounds of groundwater monitoring, further review of your
file will lead to either no further action (closeout) of your site, or, additional work may be necessary
as determined by the Division Director. This additional work may consist of performing additional
investigation activities, conducting corrective or remedial action to cleanup to established levels,
implementing groundwater or other types of environmental monitoring to assess trends in

contaminant levels, performing a risk assessment to establish site-specific cleanup levels, or other
related activities.
