The Utah Division of Waste Management and Radiation Control has reviewed your 2024 Annual Ground Water Withdrawal Survey Report, submitted on January 8, 2025. This report is required
by Part I.H.13 of the Clive Site’s Ground Water Quality Discharge Permit. UGW 450005, and Condition 12.3 of Radioactive Material License UT 2300479.
The report satisfies the requirements of and appears to be in compliance with Part I.H.13 of the Groundwater Quality Discharge Permit and Conditions 12.3 of the Radioactive Material
License. Therefore, the 2024 Annual Groundwater Withdrawal Survey requirements are closed out.
