Dear Mack Allan Straw:The Division of Drinking Water (the Division) received a request for an Operating Permit for Lower 4.0 MG Tank from your consultant, Jason Bradford and Jackson
Engineering on May 27, 2025. The Division issued Plan Approval for this project on June 1, 2022.
Our understanding of the project includes the construction of a new 4 million gallon (MG) post-tensioned reinforced concrete tank along with all appurtenances and site piping. The Lower
4 MG Tank is identified as ST011 in the Division’s database.
We have received the following information for Lower 4.0 MG Tank (ST011):
Certification of Rule Conformance with plan approval conditions by a professional engineer.
Satisfactory bacteriological results as evidence of proper disinfection and flushing.
We have determined that all conditions for operating permit issuance have been met. On this basis, an Operating Permit for Lower 4.0 MG Tank (ST011) is hereby issued as constituted by
this letter. You may now place the tank in service in your water system.
Please maintain a copy of this letter with your permanent records for future reference.
