deficiency must be corrected within 120 days of notification or have a corrective action
plan approved by DDW.

Rule: R309-515-6(12)(d)(v)Rule requires a well that pumps directly into the distribution system be equipped with
an air release/vacuum relief valve located upstream of the check valve, unless the
wellhead valve and piping provide for pumping to waste all trapped air before water is
introduced into the distribution system.

Rule: R309-545-10(1)(d)If the local authority allows discharge to a storm drain, the rule requires the drain
discharge to have a physical clearance of at least 12 inches between the discharge end of
the pipe and the overflow rim of the receiving basin. This significant deficiency must be
corrected within 120 days of notification or have a corrective action plan approved by
DDW.
Notes:Based on drawings from 1992 when the tanks were constructed, the drain line for ST003,
ST004, and ST005 end in the storm drain to the west of the facility. Will need an exception.
