Mr. Tao,
On behalf of Chevron Environmental Management Company (CEMC), AECOM has
prepared this Work Plan & Budget for Q2 2025 Groundwater Monitoring for the Chevron
Service Station No. 7-2977 located at 680 West 1800 South, Beaver, Utah.
BACKGROUND
The Site is located on the northwestern corner of 525 W and Route 160 in Beaver, Utah as
depicted in the attached Figure 5 (excerpted from the April 2025 Subsurface Investigation
Report). The closest surface water body is South Creek, located approximately 900 feet
north (upgradient) of the Site.
The Site is a Chevron-banded service station that has three gasoline underground storage
tanks (USTs) located on the northeast corner of the property and four pump islands located
in the western portion of the property. The Site has operated as a retail gasoline service
station since approximately 1955. In 1993, the five original USTs and associated piping
were removed from the Site after petroleum hydrocarbons were detected in soil and
groundwater samples collected near the USTs. Later that year, five new USTs and four
pump islands were installed in the same location as the former system. Between April 2013
and August 2019, a canopy and two additional dispenser islands were installed to the north
of monitoring well MW-4 and the UST basin.
Groundwater monitoring wells MW-1 through MW-7 were installed and sampled quarterly at
the Site between 1994 and 2002 and then monitoring was discontinued until May 2016.
Monitoring well MW-8 was installed in June 2017 to further delineate the extent of possible
down-gradient groundwater contamination. MW-9 and MW-10 were installed in October
2024 to delineate in the upgradient direction (i.e. north and northeast of MW-1 and MW-4).
PROPOSED SCOPE OF WORK
Task 1: Q3 2025 Groundwater Sampling and Reporting
AECOM will conduct one round of groundwater monitoring. A Utah-Certified Soil and
Groundwater Sampler will conduct all groundwater sampling activities. Prior to sampling,
the static water level and/or light non-aqueous phase liquids (LNAPL) measurement will be

recorded at the ten (10) site monitoring wells. LNAPL is not anticipated at the site; however,
if observed, wells in which the LNAPL thickness is greater than 0.01 feet (sheen) will not be
sampled.
Following gauging activities, each well to be sampled will be purged of approximately three
saturated casing volumes of groundwater prior to sampling. Groundwater purged from wells
for sampling activities will be contained in 55-gallon open top drums and staged on site
pending disposal.
Samples will be collected using new factory-sealed disposable bailers. Four wells were
selected for sampling because they exceeded UDEQ DERR Tier 1 Screening Criteria
during the November 2024 groundwater monitoring event. Of note, monitoring well MW-2
exceeded UDEQ DERR Initial Screening Levels (ISLs), but was not selected for sampling
because onsite downgradient wells were all below ISLs. During the Q2 2025 groundwater
monitoring event, the following wells will be sampled for laboratory analysis:
x MW-1
x MW-4
x MW-9
x MW-10
Groundwater samples will be submitted to Eurofins of Arvada, Colorado (a DERR-approved
laboratory) for analysis of:
x Methyl tert-butyl ether (MTBE), benzene, toluene, ethylbenzene, total xylenes,
and naphthalene (MBTEXN);
x Total petroleum hydrocarbons gasoline range organics (TPH-GRO) by EPA
Method 8260; and
x TPH diesel-range organics (TPH-DRO) by Environmental Protection Agency
(EPA) Method 8015 with Silica Gel Treatment.
Following collection, samples will be placed directly into appropriate lab-provided
containers. The samples will be logged onto a chain-of-custody form and stored on ice in a
cooler for transport to the laboratory for analysis.
In addition to the four groundwater samples collected from the monitoring wells, quality
assurance samples will include:
x One field duplicate; and
x One trip blank.
Lastly, one composite purge water sample will be collected for waste profiling and analyzed
for:
x TCLP RCRA 8 metals, TCLP VOCs, and TCLP SVOCs.
This task also includes management costs associated with the project. Management tasks
include the preparation of health and safety plans and workplans, reviewing project files
and historic data, scheduling, invoice review, meetings, Petroleum Storage Tank Fund
(PST) claim updates/status reports, and correspondence and contact with Chevron,
regulatory agencies and the property owner.

ESTIMATED COST AND SCHEDULE
The attached budget estimate is being submitted to DERR for approval and subsequent
reimbursement from the Petroleum Storage Tank (PST) Fund. Costs to complete the tasks
are included in Attachment A and were developed in accordance with DERR guidelines for
2025. The workplan approval application and agreement form is included in Attachment B.
The field work is tentatively scheduled for August 2025, and contingent on DERR approval.
The Groundwater Monitoring Report will be submitted to DERR within 90 days of receiving
the final analytical results.

Owner/Operator, responsible party, or other person seeking PST Trust Fund Reimbursement:
Chevron Environmental Management Company (CEMC) ("Claimant")
Utah-Certified UST Consultant: AECOM ("Contractor")
In submitting Work Plan ICY-26 and amendments thereto for the above referenced
Release ICY-26 for approval, the Claimant and the Contractor collectively, the "Parties," represent
and agree to the following:
Definitions:
"Contractor" means the person identified as such above.
"Claimant" means the person identified as such above.
“Parties” means Claimant and Contractor.
"Division Director" means the Division Director of the Division of Environmental Response and
Remediation.
“PST Fund" means the Petroleum Storage Tank Trust Fund.
"Release" means the release identified above.
"State" means the State of Utah including its agencies, officers, employees, volunteers and specifically,
the Division of Environmental Response and Remediation (DERR), the Division Director of the
Division of Environmental Response and Remediation, and the Petroleum Storage Tank Trust Fund.
"Work Plan" means the work plan identified above.
1. The key personnel, for which qualifications are submitted under R311-207-3(c), are:

2. The Claimant acknowledges that the Claimant is required to obtain a payment bond from the
Contractor under Section 14-2-1 of the Utah Code. If the Claimant fails to do so, the Claimant is
liable to each person who performed labor or services or supplied equipment or materials
("Subcontractors") in the event the Contractor does not pay the subcontractors, even if the Claimant
has paid the Contractor (Utah Code Section 14-2-2). The Claimant acknowledges and agrees that the
PST Fund will not reimburse the Claimant for such Subcontractors’ claims for payment against the
Claimant if the PST Fund has already made payment to the Claimant or Contractor. The Claimant
understands that the premium paid for a payment bond is reimbursable. Therefore, the Claimant:
a. Has required the Contractor to obtain 100 percent payment bond through a United States
Treasury-listed bonding company, and attached a copy, or;
b. Has obtained other equivalent assurance and waives all claims and remedies against the
State if the equivalent assurance does not adequately protect the Claimant.
The equivalent assurance is described as follows:
AECOM contracts with the "Claimant" and AECOM stipulates that all claims and
remedies against the State of Utah are waived.
3. The Parties agree that the Contractor shall have no cause of action against the State for payment.
The Parties acknowledge and agree that the State is not a party to any contract with the Claimant or
the Contractor for reimbursement from the PST Fund in the execution of this Work Plan, except to
the extent provided by a contract signed by the Division Director. Instead, the Claimant’s
reimbursement is governed by the Utah Underground Storage Tank Act and the Utah Underground
Storage Tank Rules. The Contractor is entitled to reimbursement solely under his/her contract with
the Claimant.
4. The Parties agree that they will use a subcontracting method consistent with the requirements of
R311-207.
5. The Parties agree that, as a condition of performing the work under the work plan, the Contractor
shall carry the insurance specified in R311-207-3(c)(4). The Contractor represents that the
Certificate of Insurance documenting the required insurance is attached or that a current certificate is
on file with the Division Director and has been provided to the Claimant. The Parties assume the
risk and responsibility of ensuring that the appropriate insurance coverage is in place.
6. The Parties agree that payments from the PST Trust Fund shall be limited to amounts that are
customary, legitimate, reasonable and consistent with R311-207. Unless the Parties and the Division
Director have entered into a written Pay-for-Performance agreement, the Parties acknowledge that
payments will be for actual time and materials expended up to, but not exceeding, the amount of the
Division Director approved work plan and Division Director approved change orders, if any.
7. The Parties shall maintain financial and operation records in sufficient detail to document all
transactions relating to PST Fund reimbursement for the execution of this work plan. The Parties
shall make available for audit and inspection all such records relating to the completion of the work
plan and related services, requirements, and expenditures until all audits initiated by State auditors
are completed, or for a period of five years from the date of PST Fund reimbursement related to the
execution of this work plan. Records which relate to disputes, litigation, or the settlement of claims
arising out of the performance of this work plan, or to cost and expenses of this work plan as to
which exception has been taken by the Division Director, shall be retained by the Parties until
disposition has been made of such disputes, litigation, claims, or exceptions.
8. The Parties certify that there is a contract between the Claimant and the Contractor for the
performance of work under the work plan for which approval is sought ("Contract") and incorporate
the provisions herein into that Contract. To the extent that the Contract or amendments thereto
conflict with any provisions herein, the provisions herein govern. The Parties shall provide the
Contract to the Division Director upon request.
9. The Division Director is a third-party beneficiary of this Petroleum Storage Tank Trust Fund Work
Plan Approval Application and Agreement and may enforce its provisions.
10. This Petroleum Storage Tank Trust Fund Work Plan Approval Application and Agreement does not
limit the Division Director’s rights and remedies under applicable law.
