Subject:Operating Permit, Rehabilitate Existing Well (WS001), System #07029, File #16343 The Division of Drinking Water (the Division) received your request for the Operating Permit
for the subject project from your consultant, Jones and Demille Engineering, on May 21, 2025. The Division issued Plan Approval for well rehabilitation on September 9, 2024. Division
staff conducted a final inspection on May 21, 2025.The project consisted of disconnecting WS002 from the system, cleaning and rehabilitating the WS001 well casing, screens, and pump,
installing a pitless adapter to raise the wellhead above the finish grade and replacing the connecting piping and appurtenances.
The Division has received the following information for the project:
Design engineer’s statement of conformance with approval conditionsand changes made during construction were in conformance with rules R309-500 through 550
As-built or record drawings.
Satisfactory bacteriological results as evidence of proper disinfection and flushing.
The Division has determined that all conditions of operating permit issuance have been met. On this basis, an Operating Permit for the existing well rehabilitation projectis hereby issued
as constituted by this letter. You may now place WS001 in service in your water system.Please maintain a copy of this letter with your permanent records for future reference.The issuance

Lacks required storage capacity
System has an exception to rule for storage, allowing the well to provide the peak instantaneous demand. This deficiency is resolved with the successful rehabilitation of WS001.

System pressure is provided by the well pump. This deficiency is resolved with the successful rehabilitation of WS001.

Unapproved source in service
When WS001 initially quit pumping, the system connected to a non-public well through temporary hoses, then drilled and connected a new well (WS002) without obtaining Plan Approval or
Operating Permit from the Division. After consultation with the Division the system decided to disconnect WS002 and rehabilitate WS001. With the removal of the temporary hoses, disconnection
of WS002 and rehabilitation and connection of WS001 this deficiency is resolved for both WS001 and WS002.
