Re: Additional Subsurface Investigation
Holiday Oil #75 Facility ID: 3000562
3047 W 300 North Release ID: OHS
West Point, Utah
Dear Mr. Cahoon:
Rockwell Solutions, Inc. has completed additional subsurface investigation activities at the above-
referenced facility in accordance with State of Utah Division of Environmental Response and
Remediation (DERR) guidelines.
INTRODUCTION
During fuel delivery on May 10, 2023, the diesel underground storage tank (UST) was overfilled
and diesel fuel was released into the tank basin. The release was reported to DERR and release
code OHS was assigned to the site.
SITE DESCRIPTION
The site is an active gas station and convenience store on the southwest corner of the 300 North/
3000 West intersection in West Point, Utah. The USTs are located on the north side of the property.
An initial Subsurface Investigation was completed and the report was submitted on November 28,
2023. Approximately 0.02 inches of free product was measured in MW-1. On January 19, 2024,
approximately 476 gallons of groundwater and free product were removed from MW-1 by a Sump
and Trap vacuum truck.
Groundwater contamination was not defined during the subsurface investigation. The purpose of
the additional investigation was to install monitoring wells to define the extent of groundwater
contamination.

METHODOLOGY
Three monitoring wells were installed on March 1, 2024. Monitoring well MW-5 was installed near
the western border of the site. Monitoring well MW-6 was installed north of MW-6 near the
northern boundary of the site adjacent to the western driveway. MW-7 was installed north, across
300 North. Well locations are shown on Figure 1. Direct Push Services was employed as the
drilling contractor under the supervision of a Rockwell Solutions representative.
Five-foot soil core samples were collected continuously from the direct push borings. A portion of
the soil core was put in a zip-lock baggie to allow hydrocarbons to volatilize. The air in the zip lock
baggie was then screened with a photoionization detector (PID). As no PID readings were detected
from any of the core samples, a laboratory sample was collected from the soil interval above the
apparent groundwater table in each boring and was submitted for laboratory analysis of MTBE,
benzene, toluene, ethylbenzene, xylenes, naphthalene (MBTEXN) total petroleum hydrocarbons-
gasoline range organics (TPH-GRO) using EPA Method 8260 and TPH-diesel range organics
(TPH-DRO) using EPA Method 8015.
Monitoring wells were installed in each boring to a depth of eighteen feet with fifteen feet of 0.010-
inch screen. The wells contain a silica sand filter around the screen and a bentonite seal above the
screen. A flush grade well protector was installed at the surface. The top-of-casing elevation at
each well was measured to the nearest 0.01 foot using a transit level.
The monitoring wells were developed and a groundwater sample was collected with a disposable
bailer. The samples were submitted for laboratory analysis of MBTEXN and TPH-GRO using EPA
Method 8260, and TPH-DRO using EPA Method 8015.
RESULTS
Soil Type
Soil type was evaluated during the subsurface investigation by field analysis of soil samples. The
native soil at the site generally consists of silty sand to approximately ten feet below grade (bg),
underlain by a layer of silty clay that extends to approximately 19.5 feet bg. Boring logs are
attached.
Soil Analytical Results
Table 1 lists the analytical results of the soil samples collected during both investigations. No
concentrations were detected in samples collected on March 1, 2024. The laboratory analytical
reports and chain-of-custody records for soil samples collected during the additional subsurface
investigation are attached. PID readings are shown on the boring logs.
Groundwater Depth and Flow
Table 2 lists the top-of-casing elevations and depths to water at the site monitoring wells.
Groundwater monitoring data indicates depth to groundwater varied from seven to eight feet below
grade. Groundwater flow on March 13, 2024, was to the southwest, as shown on Figure 1.

Groundwater Analytical Results
Table 2 lists the analytical results of the groundwater samples collected at the site. Approximately
0.01 inches of free product was present in MW-1 and a film of free product was observed in MW-4.
Samples were not collected from MW-1 and MW-4. No hydrocarbons were detected in any of the
other five monitoring wells. The laboratory analytical reports and chain-of-custody records for
samples collected during the subsurface investigation are attached.
CONCLUSIONS AND RECOMMENDATIONS
The extent of soil and groundwater contamination has been defined to ISLs at the site. Soil
contamination is limited to the area immediately around and west of the UST basin. Free product is
present in the tank basin backfill and in the native soil west of the tank basin.
Rockwell Solutions recommends continued groundwater extraction from MW-1 in the tank basin
pea gravel fill and from MW-4. Also, groundwater samples should be collected again in
approximately six months.
Please feel free to contact me should you have any questions or concerns at 801-361-2930.
Sincerely,
David S Hansen

Report Footnotes
Abbreviations
ND = Not detected at the corresponding Minimum Reporting Limit (MRL).
1 mg/L = one milligram per liter or 1 mg/kg = one milligram per kilogram = 1 part per million.
1 ug/L = one microgram per liter or 1 ug/kg = one microgram per kilogram = 1 part per billion.
1 ng/L = one nanogram per liter or 1 ng/kg = one nanogram per kilogram = 1 part per trillion.
On calculated parameters, there may be a slight difference between summing the rounded values shown on the report
vs the unrounded values used in the calculation.
