Subsurface Investigation Report - UDOT Santaquin Maintenance Station #3424 WSP Project No. 2381400503 July 2023 Page 1 EXECUTIVE SUMMARY This subsurface investigation report (SIR) details the results of the soil sampling activities performed in March 2023 by WSP USA Environment and Infrastructure Inc. (WSP). This SIR was completed in response to the petroleum release identified during the November 2022 system upgrade for two aboveground petroleum storage tanks by Spackman Enterprises, Inc (Spackman). On November 22, 2022, Spackman removed one fuel dispenser island and underground product piping, in accordance with DERR approved closure plan. Based on the review of laboratory analytical results of closure soil samples included in Spackman’s Closure Notice, the Utah DERR identified a suspected release and assigned it Release ID OGK. DERR determined that further investigation of the site was needed to determine the extent of contamination. In January 2023, the Utah DERR issued a letter requesting that an SIR be prepared for the site. In response, WSP submitted a Site Investigation Work Plan, which was approved by the DERR by March 2023. WSP completed the subsurface investigation (SI) by installing 13 soil borings in the vicinity of the former underground product piping and dispensers island and collecting soil samples to determine the extent of contamination. Soil samples collected at the Site were submitted for analysis of petroleum hydrocarbon parameters. None of the analytical parameters in any of the samples was found at concentration levels above the respective Initial Screening Levels. Additionally, no free product or groundwater were encountered during advancement of soil borings up to a maximum depth of 25 feet below ground surface (bgs). Based on the results of this investigation it was concluded that the release has been adequately delineated both horizontally and vertically with no impact to groundwater. Petroleum hydrocarbons impacted soil may still be present at the Site at the vicinity of the former underground product piping at a depth of approximately 3 feet bgs. It is recommended that an Environmental Covenant be placed on the site to manage impacted soils and the release be closed with a no further action. Subsurface Investigation Report - UDOT Santaquin Maintenance Station #3424 WSP Project No. 2381400503 July 2023 Page 2 1.0 INTRODUCTION WSP USA Environment & Infrastructure Inc. (WSP) was retained by the Utah State Fuel Network to complete a Subsurface Investigation Report (SIR) for the Utah Department of Transportation (UDOT) Santaquin Maintenance Station No. 3424 (“Site”), Facility Identification No. 1000796, located at 637 W Highway 6 in Santaquin, Utah. The Site vicinity is shown on Figure 1. On December 9, 2022, a release of petroleum from an aboveground storage tank (AST) or petroleum-carrying pipelines at the Site was reported to the Utah Department of Environmental Quality (UDEQ) Division of Environmental Response and Remediation (DERR) based on sampling results obtained during system modification activities. The release at the Site was assigned Release Site OGK. 1.1 SITE HISTORY Based on the information reviewed from the UDEQ DERR, one fuel dispenser and underground product piping were removed on November 22, 2022, in accordance with UDEQ DERR approved closure plan. One 4,000-gallon gasoline AST and one 2,000-gallon diesel AST remain in use at the Site, and new aboveground piping was installed. The dispenser island was removed, and a new island installed to the south of its original position. Site features including the locations of the former AST components, ASTs, and utilities are shown on the Site Map provided in Figure 2. Closure soil sample SS-1 was collected below the dispenser island at a depth of two feet below grade level (bgs) and SS-2 was collected below the product piping at a depth of three feet bgs. After receiving laboratory results for the closure samples, Spackman notified the DERR of a product release at the Site. On January 5, 2022, Spackman Enterprises submitted to DERR, an UST Permanent Closure Notice. A concentration of total petroleum hydrocarbons (TPH) diesel range organics (DRO) was detected in SS-1 at 31,700 milligrams per kilogram (mg/kg), above the DERR Initial Screening level (ISL). TPH gasoline range organics (TPH-GRO), TPH-DRO, ethylbenzene, and xylenes were detected in SS-2 (3) at Concentration levels of 1,470 mg/kg, 6,560 mg/kg, 34.3 mg/kg, 249 mg/kg, respectively above their respective ISLs (Spackman, 2022). The DERR issued a letter on January 26, 2023 (ERRL-0014-23), requesting that a site investigation report (SIR) be completed for release OGK. WSP submitted a Site Investigation Work Plan to the State Fuel Network on March 9, 2023, to determine the horizontal and vertical extent of contaminated soil from the area of the removed product piping and dispenser island. Groundwater investigation was not included in this SIR report based on the depth to groundwater in the area being approximately 80 feet bgs. 1.2 SUMMARY OF WORK COMPLETED WSP conducted a review of historical releases, geological/hydrological conditions, and online databases of the Site. This research was completed to determine the extent of any environmental impacts on the site. WSP conducted a subsurface investigation (SI) in the vicinity of the former dispenser island and petroleum product piping to define the subsurface extent and degree of soil contamination associated with the petroleum release. Thirteen soil borings were installed at the Site in the vicinity Subsurface Investigation Report - UDOT Santaquin Maintenance Station #3424 WSP Project No. 2381400503 July 2023 Page 3 of the AST system. WSP collected soil samples from the borings for laboratory analysis of benzene, toluene, ethylbenzene, xylene, naphthalene (BTEXN), methyl tert-butyl ether (MTBE) by EPA Method 8260 and TPH-DRO by EPA Method 8015. One soil sample was also submitted for TPH fractionation by EPA Method 8270. 2.0 SITE DESCRIPTION AND MAPS The Site is referenced as UDOT Santaquin Maintenance Station #3424 and is located at 637 W Highway 6 in Santaquin, Utah, and is approximately 8.89 acres. The approximate geographic coordinates for the Site are Latitude North 39 degrees 58 minute and 39 seconds and Longitude West 111 degrees 47 minutes and 57 seconds located in Section 2, Township 10S, and Range 1E Salt Lake Principal Meridian. The Site has been used by UDOT since 1995 for road maintenance operations in the area. The Site layout is shown on Figure 2. Currently the Site is improved with a 10,500 Square Foot (sqft) building used for offices, vehicle storage and maintenance and a 2,400 sqft storage shed for salt. One gasoline and one diesel ASTs are located on the north side of the Site within the fenced area. Other improvements include a truck wash out area and a retention pond. The remainder of the Site is used for storage of road materials as needed. Approximately 95% of the Site’s surface area is paved. 3.0 TIER 1 CRITERIA The current land use at the Site is commercial and public access is restricted by a fence. A commercial property is located to the northeast of the property beyond W. Main Street and the remaining surrounding properties are agricultural properties. Single-family residential properties are located beyond the agricultural land to the north/northeast, southwest, and east at approximate distance of 900 feet, 1,500 feet, 860 feet respectively. In addition, a church is located approximately 700 feet to the southeast beyond the agricultural properties. 3.1 UTILITIES The property is serviced by the following utilities: electrical, water, sewar, natural gas, and communication. A private locate of the utilities in the vicinity of the ASTs was conducted at the Site on March 24, 2023. The private locate identified electrical, natural gas, and communication services along Main Street at the property’s north boundary. The locate confirmed that the electrical and natural gas utilities connect to the building in the vicinity of the ASTs, however the communication connection wasn’t identified. The sewar service enters the property on the south property boundary and connects to the south side of the building. The water service was not identified during the located and is assumed to be outside of the survey area, however a water valve was identified located adjacent to the former dispenser island. An unknown utility was identified between the building and the ASTs, however the utility could only be traced a short distance. The utilities in the vicinity of the ASTs are assumed to be buried approximately 2 to 4 feet below grade surface (bgs). The depth of the sewer and water lines are unknown however the depth is Subsurface Investigation Report - UDOT Santaquin Maintenance Station #3424 WSP Project No. 2381400503 July 2023 Page 4 assumed to be less than 10 feet bgs. No utilities intersect water table. Utility locations are shown on Figure 2. 3.2 SITE SOIL TYPE One soil sample (USC-1 2’) was collected below the dispenser island at a depth of two feet bgs and submitted for analysis for Unified Soil Classification (USC) during the November 2022 UST closure. The USC description of the encountered soil is (GC) Clayey Gravel or gravel-sand-clay mixtures. Soil samples collected during the March 2023 SI were collected from soil borings at a shallow depth between five and 10 feet bgs and at the source area boring at depths of 14.5-15 and 24.5-25 feet bgs. Soil encountered onsite from surface to 10 feet bgs consisted of clayey silt and 10 to 25 feet bgs consisted of silty subrounded gravel with diameters of gravels approximately 1 inch in diameter. Soil boring logs are provided in Appendix A. 3.3 GROUNDWATER DEPTH AND FLOW DIRECTION Based on the UST closure report, groundwater was not encountered in the excavations during the 2022 UST closure activities. During the March 2023 SI, slightly moist soils were observed in some of the borings at a depth of between five and ten feet bgs, however no groundwater or free product were encountered. Based on information found on the UDEQ Environmental Database from previous assessment activities for sites located in the town of Santaquin, the depth to groundwater in the vicinity of the Site is variable and is at a depth between 80 to 400 feet bgs. Records of the two release sites are summarized below and included as Appendix B. • The old UDOT Station #624, Facility ID 1000335/Release HMI, located approximately 2,364 feet to the southeast of the Site - the release records indicated depth to groundwater of approximately 80 feet bgs. • The Santaquin City Corp site, Facility ID 1000536/Release HNN, located approximately 2,300 feet south-southeast of the Site - the release records indicated depth to groundwater between 400-500 feet bgs. A review of the Points of Diversion in the vicinity of the Site using existing Utah Division of Water Rights (DWR) database indicated a water well (51-2965) located approximately 1,490 feet northwest of the site had a static depth to water in 1974 at 60 feet bgs (Appendix C). In addition, the Cedar Hollow Reservoir is located southwest of the Site and its nearest point to the Site is approximately 4,800 feet. Based on the information from the database review, groundwater monitoring activities were not included in this SI. 3.4 DISTANCE TO UTILITIES AND PROPERTY BOUNDARIES With depth to groundwater in the area of the Site approximately 60 to 400 feet bgs, all utilities are significantly above the water table. The table below shows the distance of utilities, building, Subsurface Investigation Report - UDOT Santaquin Maintenance Station #3424 WSP Project No. 2381400503 July 2023 Page 5 property lines from the former dispenser island and former product piping transition box which were identified as the release areas. The nearest utility to the release areas are the electrical line and water valve located adjacent to the former dispenser island approximately 5 feet to the west. 3.5 WATER WELL SURVEY A review of existing Points of Diversion in the vicinity of the Site was performed using existing Utah Division of Water Rights maps and data 1. One water well was identified within a 1,500-foot radius of the site: well 51-2965 is located approximately 1,490 feet northwest of the site. Based on the well log the well total depth is 253 feet bgs with the pump intake set at 194 feet bgs. Static depth to water is recorded at 60 feet bgs in 1974. Seven additional wells are present beyond the 1,500-foot radius of the site to the northwest, north and northeast. DWR Points of Diversion Map is included in Appendix C. 1 The database was accessed on June 27, 2023 Subsurface Investigation Report - UDOT Santaquin Maintenance Station #3424 WSP Project No. 2381400503 July 2023 Page 6 4.0 NATURE OF THE RELEASE AND ABATEMENT MEASURES 4.1 RELEASE INFORMATION The release was identified by analytical results of closure soil samples collected during AST system modification activities including removal of underground product piping and dispenser island. The source and location of the release from the AST system was confirmed to be from the underground product piping and dispenser island. Based on the UST Closure Report the following information was obtained: Description Fuel type Volume (gal.) Date Installed Date Closed AST Gasoline 4000 12/1/2000 Current AST Diesel 2000 12/1/2000 Current No water or soil was removed from the excavation during AST system modification activities. 4.2 AST CLOSURE SAMPLE RESULTS On November 22, 2022, underground product piping and dispenser island were removed. On January 5, 2023, Spackman Enterprises submitted to the DERR, an UST Permanent Closure Notice. A concentration of total petroleum hydrocarbons (TPH) diesel range organics (DRO) was detected in SS-1 (2) above the Utah Initial Screening level (ISL) at 31,700 milligrams per kilogram (mg/kg). Concentrations gasoline range organics (GRO), TPH-DRO, ethylbenzene, xylenes were detected in SS-2 (3) above the Utah ISLs at 1,470 mg/kg, 6,560 mg/kg, 34.3 mg/kg, 249 mg/kg, respectively (Spackman, 2022). No abatement measures of the soil were conducted by Spackman Enterprises. Excavated overburden soil was placed back in the excavation following system component removal. 5.0 SI METHODOLOGY Prior to Site activities, Blue Stakes was contacted by WSP to have public utilities located/marked on, and near, the Site. In addition, a private utility locating sub-contractor conducted a ground penetrating radar survey to identify onsite utilities and clear each proposed boring location. A total of thirteen soil borings were advanced onsite on March 27, 2023 (Figure 3). Drilling activities were completed by ET Technology (ET) using direct push technology. Soil cores were retrieved in continuous 5-foot intervals in acetate liners using dual tube macro cores. Dual tube macro core technology was used to aid core recovery for logging and field screening. Soil boring SB-1 was advanced to a total depth of 25 feet bgs and SB-2 through SB-13 were advanced to total depths between 9.5 feet and 10 feet. Borings SB-1 and SB-11 were installed in the source areas identified during the UST closure sampling. During installation of borings, the soil cores were immediately screened with a photoionization detector (PID) to determine zones of soil contamination. Soil samples were collected from each boring to delineate the horizontal and vertical soil contamination around the AST system. Subsurface Investigation Report - UDOT Santaquin Maintenance Station #3424 WSP Project No. 2381400503 July 2023 Page 7 5.1 SAMPLING METHODOLOGIES 5.1.1 Soil Lithology According to the United States Department of Agriculture, Soil Conservation Service, Web Soil Survey, digital data is available for soils underlying the Site. The present soil at Site is described as 65% Steed gravely sandy loam, 17 % Steed sandy Loam and 18% minor components. The Steed gravely sandy loam consists of 0 to 3 percent slopes with a typical soil configuration of gravely sandy loam clay loam from 0 to 7 inches, extremely gravelly loamy sand from 7 to 31 inches, extremely gravely sand from 31 to 60 inches. Based on soil borings installed at the Site, the underlying stratigraphy is clayey silt overlying silty gravel at depth. The gravels consist of subrounded quartz that is approximately 1-inch diameter. Soil lithology is described on the boring logs included in Appendix A. 5.1.2 Soil Sampling PID readings were collected from soil borings SB-1 through SB-13 and recorded on the soil boring logs (Appendix A). Low PID readings ranging from 1 to 8 ppm were recorded in SB-3 and SB-11 at depths ranging from 2 to 7 feet bgs. The remaining borings were all 0.0 PPM. Odors were also noted in the soil cores from borings SB-3 and SB-11; however, no soil staining was encountered in any of the soil borings. Field PID readings were used to adjust the location of the soil sampling for each core, soils were collected from the highest screened location of the soil cores in SB-3 and SB-11. A total of 27 soil samples were collected from the 13 soil borings. Three soil samples were collected from the source area boring (SB-1), and two soil samples were collected from each of the remaining borings SB-2 through SB-13. In each boring location, one soil sample was collected at an approximate depth of 4.5 to 6.5 feet bgs, immediately below the assumed base of the former product piping and/or dispenser island. A second sample was collected approximately five feet below the shallower sample for vertical characterization. An additional (third) soil sample was collected at 24.5-25 feet bgs from the assumed source area boring location SB-1. However, due to limited space between the fence and the current AST, a hand auger was utilized for sampling at the second source area boring location (SB-11). Therefore, only two soil samples were collected at SB-11 as described above. Soil samples were placed directly into new, clean, glass sample containers provided by the analytical laboratory. Sample containers for volatile organics analysis were filled completely to eliminate headspace. Following collection, each sample was immediately labeled with the WSP project number, date, and time of sample collection, and sample identification number. Samples were placed in a cooler and chilled to approximately 4° Celsius. Chain-of-custody forms were completed on-site to document delivery of the samples to the analytical laboratory. Samples were delivered via FedEx to Pace National in Mt. Juliet, Tennessee for analysis of TPH-DRO by EPA Method 8015 and TPH-GRO and MBTEXN by EPA Method 8260. One soil sample collected from SB-11 was delivered to ChemTech-Ford in Sandy, Utah for analysis of TPH Fractionation by EPA Method 8270. Subsurface Investigation Report - UDOT Santaquin Maintenance Station #3424 WSP Project No. 2381400503 July 2023 Page 8 5.2 DECONTAMINATION AND INVESTIGATIVE DERIVED WASTE Boring and sampling equipment were decontaminated using an Alconox® wash, followed by potable water and water rinses. Samples were collected using clean nitrile gloves for each sample. Disposal sampling equipment was bagged and disposed off-site as non-hazardous waste. Following drilling activities, the soil cuttings were placed back into the boring and capped with bentonite and concrete. 6.0 RESULTS 6.1 GEOLOGY AND SOIL HYDROCARBON RESULTS 6.1.1 Geologic Background According to the Utah Geological Survey, the site is located in the Santaquin Quadrangle. The subsurface material present on Site is unconsolidated material consisting of Offshore Deposits of the Bonneville Lake Cycle (Qbo) Unit deposited during the Pleistocene Age. The material consists of light gray to light tan, well sorted and even-bedded deposits of clay, silt, and some sand derived from a lacustrine origin. The thickness of the deposited layers is uncertain and varies. In addition, the subsurface material underlying part of the Town of Santaquin is the Nearshore Deposits of the Bonneville Lake Cycle (Qbn) Unit deposited during the Pleistocene Age. The material consists of light gray to gray, moderately well sorted, even-bedded deposits of cross bedded silt, sand, gravel, and sparse cobbles derived from a deltaic origin. The thickness of the deposited layers are vary and may be as much as 250 feet. 6.1.2 Soil Hydrocarbon Analysis A total of 27 soil samples were collected from 13 soil borings (SB-1 through MW-13) and were analyzed for TPH-DRO by EPA Method 8015 and TPH-GRO and MBTEXN by EPA Method 8260B. by EPA Method 8270. The analytical results were compared to the Utah ISLs and are summarized in Table 1. Copies of the Laboratory analytical reports for soil samples collected during the March 2023 sampling event are provided in Appendix D. Subsurface Investigation Report - UDOT Santaquin Maintenance Station #3424 WSP Project No. 2381400503 July 2023 Page 9 Detectable concentrations of TPH-DRO below the ISL were reported in all soil samples. In addition, detectable concentrations of benzene, ethylbenzene, xylenes, and naphthalene below the Utah ISLs were reported in SB-11 (4.5-5), SB-11 (9-9.5), SB-13 (4.5-5), and SB-13 (9.5-10). One soil sample from SB-11 was submitted for TPH Fractionation. Low concentration levels of C11-C12, C13-C16, C17-C21, and C22-C35 aliphatic hydrocarbons, and chrysene were reported in the sample. The fractionation analysis results are summarized in Table 2. 6.2 HYDROGEOLOGY AND GROUNDWATER HYDROCARBON RESULTS 6.2.1 Hydrogeologic Background Two water reservoirs are located west of the Site approximately 1,900 feet and 2,600 feet, respectively. The Cedar Hollow Reservoir is located approximately 4,800 feet southwest of the Site and is most likely connect to the two reservoirs to the north by an intermittent stream. The site is located in the Utah Lake hydrologic unit which is included in the Great Salt Lake hydrologic subregion. Groundwater at the Site is expected to flow in unconfined aquifers in unconsolidated soils. Onsite monitoring wells were not installed during the March 2023 SI. 6.2.2 Groundwater Analysis Groundwater was not encountered in the soil borings and was not included in this assessment. 6.3 QUALITY ASSURANCE The following sections discuss the Quality Assurance/Quality Control (QA/QC) measures utilized for laboratory analysis at the Site. Based on the laboratory report, none of the data was rejected based on QA parameters, and the data is considered valid for the intended use. 6.3.1 Field QA/QC Sampling During field activities, a field duplicate sample (DUP-032723) was collected from the parent sample SB-1 (5-5.5) and was analyzed for TPH-DRO by EPA Method 8015 and TPH-GRO and MBTEXN by EPA Method 8260B. DUP sample analytical results are provided in Table 1. A relative percent difference (RPD) was not calculated as all analytes were below the laboratory reporting limits. A trip blank sample was included with the sample cooler during transportation and was analyzed for MBTEXN by EPA Method 8260B. Laboratory results are provided in Table 3. The analytical results for all analytes were below the laboratory detection limits for the analytes. 6.3.2 Internal Laboratory QA/QC Laboratory QA/QC procedures, including method blank analyses, Laboratory Control Sample (LCS) recoveries, and surrogate recoveries, were evaluated and are provided in laboratory reports (Appendix D). Subsurface Investigation Report - UDOT Santaquin Maintenance Station #3424 WSP Project No. 2381400503 July 2023 Page 10 The laboratory performance criteria evaluated during internal laboratory data review included initial and continuing calibration data, LCS results, and other checks pertinent to specific analytical methods (e.g., internal standard and inductively coupled plasma interference check sample data). 6.3.3 Holding Times Analyses were conducted within the required holding times. There were no flags associated with holding times. 6.3.4 Method Blanks WSP reviewed the laboratory method blank reports to evaluate the potential bias in sample results due to blank contamination. No bias was noted in the sample reports. 6.3.5 Laboratory Control Sample The accuracy of the instrument calibration, recovery during sample preparation, and long-term stability are checked by the analysis of LCS with each batch. The LCS results were within the control limits, indicating that the methods were in control. 7.0 CONCLUSIONS AND RECOMENDATIONS On November 22, 2022, fuel system modifications were completed which included the removal of underground product piping and replacement with aboveground piping; and relocating of the dispensers. Based on the closure soil samples a release was identified at two locations near the piping transition box and near the connection to the dispensers. In March 2023 WSP completed a SI at the Site to delineate the horizontal and vertical extent of soil contamination. Thirteen soil borings were installed at the source area and adjacent areas, and soil samples were field screened and submitted for laboratory analysis. None of the analytical parameters exceeded the respective ISLs in any of the samples collected. No free product was observed in the soils during the SI soil sampling activities and no groundwater was encountered in the boring installed at a maximum total depth of 25 feet bgs. Based on the SI soil samples analytical results, WSP believes that the release has been adequately delineated, both horizontally and vertically. Limited amount of soil impacted by petroleum compounds may still be present at the Site at the vicinity of the former underground product piping, at a depth of approximately 3 feet bgs. WSP recommends that Environmental Covenant be placed on the site to manage impacted soils and the release be closed with a no further action. This is not intended as a comprehensive explanation, and if you have additional questions please contact your project representative. Results Disclaimer - Information that may be provided by the customer, and contained within this report, include Permit Limits, Project Name, Sample ID, Sample Matrix, Sample Preservation, Field Blanks, Field Spikes, Field Duplicates, On-Site Data, Sampling Collection Dates/Times, and Sampling Location. Results relate to the accuracy of this information provided, and as the samples are received. Abbreviations and Definitions (dry)Results are reported based on the dry weight of the sample. [this will only be present on a dry report basis for soils]. MDL Method Detection Limit. MDL (dry)Method Detection Limit. RDL Reported Detection Limit. RDL (dry)Reported Detection Limit. Rec.Recovery. RPD Relative Percent Difference. SDG Sample Delivery Group. (S) Surrogate (Surrogate Standard) - Analytes added to every blank, sample, Laboratory Control Sample/Duplicate and Matrix Spike/Duplicate; used to evaluate analytical efficiency by measuring recovery. Surrogates are not expected to be detected in all environmental media. U Not detected at the Reporting Limit (or MDL where applicable). Analyte The name of the particular compound or analysis performed. Some Analyses and Methods will have multiple analytes reported. Dilution If the sample matrix contains an interfering material, the sample preparation volume or weight values differ from the standard, or if concentrations of analytes in the sample are higher than the highest limit of concentration that the laboratory can accurately report, the sample may be diluted for analysis. If a value different than 1 is used in this field, the result reported has already been corrected for this factor. Limits These are the target % recovery ranges or % difference value that the laboratory has historically determined as normal for the method and analyte being reported. Successful QC Sample analysis will target all analytes recovered or duplicated within these ranges. Original Sample The non-spiked sample in the prep batch used to determine the Relative Percent Difference (RPD) from a quality control sample. The Original Sample may not be included within the reported SDG. Qualifier This column provides a letter and/or number designation that corresponds to additional information concerning the result reported. If a Qualifier is present, a definition per Qualifier is provided within the Glossary and Definitions page and potentially a discussion of possible implications of the Qualifier in the Case Narrative if applicable. Result The actual analytical final result (corrected for any sample specific characteristics) reported for your sample. If there was no measurable result returned for a specific analyte, the result in this column may state “ND” (Not Detected) or “BDL” (Below Detectable Levels). The information in the results column should always be accompanied by either an MDL (Method Detection Limit) or RDL (Reporting Detection Limit) that defines the lowest value that the laboratory could detect or report for this analyte. Uncertainty (Radiochemistry)Confidence level of 2 sigma. Case Narrative (Cn) A brief discussion about the included sample results, including a discussion of any non-conformances to protocol observed either at sample receipt by the laboratory from the field or during the analytical process. If present, there will be a section in the Case Narrative to discuss the meaning of any data qualifiers used in the report. Quality Control Summary (Qc) This section of the report includes the results of the laboratory quality control analyses required by procedure or analytical methods to assist in evaluating the validity of the results reported for your samples. These analyses are not being performed on your samples typically, but on laboratory generated material. Sample Chain of Custody (Sc) This is the document created in the field when your samples were initially collected. This is used to verify the time and date of collection, the person collecting the samples, and the analyses that the laboratory is requested to perform. This chain of custody also documents all persons (excluding commercial shippers) that have had control or possession of the samples from the time of collection until delivery to the laboratory for analysis. Sample Results (Sr) This section of your report will provide the results of all testing performed on your samples. These results are provided by sample ID and are separated by the analyses performed on each sample. The header line of each analysis section for each sample will provide the name and method number for the analysis reported. Sample Summary (Ss)This section of the Analytical Report defines the specific analyses performed for each sample ID, including the dates and times of preparation and/or analysis. Qualifier Description J The identification of the analyte is acceptable; the reported value is an estimate. J3 The associated batch QC was outside the established quality control range for precision. J4 The associated batch QC was outside the established quality control range for accuracy. J5 The sample matrix interfered with the ability to make any accurate determination; spike value is high. J6 The sample matrix interfered with the ability to make any accurate determination; spike value is low. 1 Cp 2 Tc 3 Ss 4 Cn 5 Sr 6 Qc 7 Gl 8 Al 9 Sc ACCOUNT:PROJECT:SDG:DATE/TIME:PAGE: WSP (Formely Wood E&I)-UT 2381400503 L1599279 04/10/23 16:25 54 of 59 ACCOUNT:PROJECT:SDG:DATE/TIME:PAGE: WSP (Formely Wood E&I)-UT 2381400503 L1599279 04/11/23 08:52 54 of 59 Pace Analytical National 12065 Lebanon Rd Mount Juliet, TN 37122 Alabama 40660 Nebraska NE-OS-15-05 Alaska 17-026 Nevada TN000032021-1 Arizona AZ0612 New Hampshire 2975 Arkansas 88-0469 New Jersey–NELAP TN002 California 2932 New Mexico ¹TN00003 Colorado TN00003 New York 11742 Connecticut PH-0197 North Carolina Env375 Florida E87487 North Carolina ¹DW21704 Georgia NELAP North Carolina ³41 Georgia ¹923 North Dakota R-140 Idaho TN00003 Ohio–VAP CL0069 Illinois 200008 Oklahoma 9915 Indiana C-TN-01 Oregon TN200002 Iowa 364 Pennsylvania 68-02979 Kansas E-10277 Rhode Island LAO00356 Kentucky ¹ ⁶KY90010 South Carolina 84004002 Kentucky ²16 South Dakota n/a Louisiana AI30792 Tennessee ¹ ⁴2006 Louisiana LA018 Texas T104704245-20-18 Maine TN00003 Texas ⁵LAB0152 Maryland 324 Utah TN000032021-11 Massachusetts M-TN003 Vermont VT2006 Michigan 9958 Virginia 110033 Minnesota 047-999-395 Washington C847 Mississippi TN00003 West Virginia 233 Missouri 340 Wisconsin 998093910 Montana CERT0086 Wyoming A2LA A2LA – ISO 17025 1461.01 AIHA-LAP,LLC EMLAP 100789 A2LA – ISO 17025 ⁵1461.02 DOD 1461.01 Canada 1461.01 USDA P330-15-00234 EPA–Crypto TN00003 ACCREDITATIONS & LOCATIONS ¹ Drinking Water ² Underground Storage Tanks ³ Aquatic Toxicity ⁴ Chemical/Microbiological ⁵ Mold ⁶ Wastewater n/a Accreditation not applicable * Not all certifications held by the laboratory are applicable to the results reported in the attached report. * Accreditation is only applicable to the test methods specified on each scope of accreditation held by Pace Analytical. 1 Cp 2 Tc 3 Ss 4 Cn 5 Sr 6 Qc 7 Gl 8 Al 9 Sc ACCOUNT:PROJECT:SDG:DATE/TIME:PAGE: WSP (Formely Wood E&I)-UT 2381400503 L1599279 04/10/23 16:25 55 of 59 ACCOUNT:PROJECT:SDG:DATE/TIME:PAGE: WSP (Formely Wood E&I)-UT 2381400503 L1599279 04/11/23 08:52 55 of 59 The analyses presented on this report were performed in accordance with the National Environmental Laboratory Accreditation Program (NELAP) unless noted in the comments, flags, or case narrative.
