Re:Extension for Corrective Action
Former Smith’s Gas and Video, 905 West North Temple Street, Salt Lake City
Facility ID 4000251, Release IUQ
Dear Mr. Davis:
The Division of Environmental Response and Remediation (DERR) received an email on
April 1, 2025, from Wasatch Environmental, providing an update on the status of the redevelopment
project and revised schedule for implementing the Corrective Action Plan (CAP). Wasatch
Environmental has requested an extension for the CAP implementation and reporting on behalf of
J Fisher Companies.
I have reviewed the request and agree to extend the deadline specified in the Corrective
Action Plan Approval letter dated September 28, 2024. The DERR understands that J Fisher
Companies anticipates CAP activities will begin on or before July 2025. Please contact the DERR
as schedules are finalized. If corrective action has not begun by September 30, 2025, a revised CAP
or CAP extension request must be submitted for approval.
Please submit a corrective action report detailing the cleanup activities and any sample
results within 60 days after corrective action is complete. Once it has been adequately demonstrated
that contaminant concentrations are below the established cleanup levels, you may petition the
DERR for site closure. After reviewing this information, the DERR will determine if no further
action is warranted or if additional work is needed.
