Jenkins Oil, 395 South State Street (Highway 89), Orderville
Facility ID 6000782, Releases ONB and OIA
Dear Mr. Jenkins:
A release from a petroleum storage tank (PST) at the above-referenced site was reported on
March 24, 2025, to the Division of Environmental Response and Remediation (DERR) based on
Spill Report Number 19564. This release is assigned as Release ONB. According to Utah
Administrative Code R311-202, you are required to report, control, abate, and characterize the
release by defining the extent and degree of petroleum contamination and remediate (clean up) the
release. These regulations require that you prepare specific reports addressing the release and submit
them to the DERR within specified time frames following the release.
The DERR project manager reviewed the release report information and visited the site on
March 28, 2025, to observe the abatement excavation of impacted soil. A report outlining any
abatement activities related to Release ONB must be submitted to the DERR within 30 days of your
receipt of this letter. Based on the extent of the release, the project manager will determine if further
assessment may be required.
According to site personnel, the center diesel tank involved in the release was recently
removed from service due to a suspected ongoing release. The tank was not locked out or labeled
to prevent petroleum deliveries, which contributed to the release. Please take immediate measures
to disable or lock out the fill port of the tank and label the tank “out of service” to prevent any future
incidents. Moving forward, this tank should be closed and removed from the facility or recertified
by the manufacturer before receiving any petroleum deliveries.

An approved Corrective Action Plan (CAP) is underway for Release OIA at the site and
should continue as planned. If adjustments are necessary to account for impacts related to Release
ONB, your consultant should work with the DERR project manager to coordinate efforts.
Section 19-6-420(10)(a) of the Underground Storage Tank Act allows the DERR to recover
certain expenses incurred for petroleum releases not covered by the PST Fund. The DERR’s
expenses for managing and overseeing Release ONB will be billed under the existing oversight
agreement for Release OIA at the Jenkins Oil Facility 6000782.
