Dear Ms. Chavez:
On January 13 and 14,2025,a Hamilton Environmental representative collected groundwater data
from eighteen (18) monitoring wells at the Petro Mart V (Hoagies) site located at2705 North 2000
West, Farr West, Utah. Appendix A - Figure I shows Site Map with approximate sampling
locations of the monitoring wells.
Collection of Groundwater Samples and Data
Prior to sampling, fluid level measurements were collected from Monitoring Wells 7-13,15-17,
19 and 20 Monitoring. Well 14 was destroyed during road construction and Monitoring Well 18
was not installed. Depth-to-groundwater readings are listed in Appendix A - Table 1. No Free-
product was observed in the 18 wells.
The 18 wells were purged of three well volumes of groundwater with a pump. These monitoring
wells were sampled and the groundwater placed in the appropriate 40 ml glass bottles. The samples
were placed on ice and transported under chain-of-custody to Chemtech-Ford Laboratories.
Chemtech-Ford Laboratories is a Laboratory Accredited Program (I.{ELAP) in Utah. The
groundwater samples were analyzed for MTBE, benzene, toluene, ethylbenzene, xylenes,
naphthalene (MBTEXN), and total petroleum hydrocarbons (TPH) analysis employing EpA
Method 8260 D, 50304, gZ6OBlC, and 8015C.
The average groundwater depth below ground surface from October 7-8, 2024 to January 13-14
decreased approximately 2.11 feet. The Octob er 7-8,2024, sampling event was conducted when
there was no irrigation water in the adjoining fields and after a relatively light water summer. TheJanuary 7-8, 2024, sampling event was conducted when there was no inigation water in the
adjoining fields' See Appendix A - Table 1. Groundwater contours can be reviewed in Appendix
A - Figure 2'

Recommendations - Groundwater Sampling
Hamilton Environmental recommends that the groundwater in the Petro Mart-5 site be monitored

Has DERR reviewed the previous Work Plan Report and made a decision to replace the Ie AirFiltration system?
We would recommend that the owner continues to arrange to have the 30 plus year old tanks be
replaced and the petroleum contamination in the tank farm basin be remediated.
