Dear Mr. Bird,
In compliance with the reporting requirements of 40 CFR Part 63 Subpart CC, the following constitutes
the periodic report required by $63.655(9) applicable to the Chevron Salt Lake Refinery. This submittal
covers the period from July 19,2024 to January 18,2025.
The following sections include the applicable reporting requirements.
I. Internal Floating Roofs - $63.655(9)(2)
This requirement is not applicable as there are no IFR tanks subject to $63.646 or $63.660 at
the Refinery.
II. External Floating Roofs - $63.655(gX3)
The requirements of $63.655(g)(3Xi) do not apply as there are no EFR tanks subject to
$63.646 at the Refinery. For purposes of $63.655(gX3Xii), there were no EFR tank inspection
failures or extension requests utilized during the reporting period.
III. Miscellaneous Process Vents - $63.655(gX6)
There were no deviations during the reporting period.
IV. Performance Test Results - $63.655(gX7)
Not applicable for the reporting period. No performance tests for purposes of 40 CFR 63
Subpart CC were conducted during the reporting period.
V. Heat Exchange Systems - $63.655(g)(9)
(i) The number of heat exchange systems at the plant site subject to the monitoring
requirements in $63.654: four
. The number of heat exchange systems at the plant site found to be leaking: zero
Monitoring is conducted monthly in accordance with $63.65a(c)(a)(i), therefore the
total strippable hydrocarbon leak action level is 6.2 ppmv.

VI.Pressure Relief Devices (PRD) - $63.655(gX10)
During the reporting period, there were no instrument readings of 500 ppm or greater as
detected by Method 21 of 40 CFR Part 60. All required PRD monitoring was conducted
during the reporting period.
VII. Flares - $63.655(gXll)
(i) For purposes of $63.655(gX I I Xi), there were no instrument readings of at least one
minute of regulated material being routed to the flare with no pilot flame present
during the reporting period.
(ii) For purposes of $63.655(eX I I )(ii), there were instances of visible emissions that
exceeded a total of 5 minutes in a 2-hour period during the reporting period. Each
instance is shown in the table below.

( iii)During the reporting period, for purposes of $63.655(gX I I Xiii), there were no flare
tip velocity deviations from the limits at $63.670(dXl)-(2).The net heating value in
the combustion zone (NHVcz) for Flares 1,2 and 3 periodically dropped below the
threshold at $63.670(e)( I ) of 270 Btu/scf. Each I 5-minute block below the threshold
is shown in the tables below.

VIII. Delayed Coking Units - S63.655(gXl2)
For purposes of $63.655(g)(12)(i), there were no deviations from the limits at $63.657(a)(l)
during the reporting period. For purposes of $63.655(gX I 2Xiii), there were no double-
quenching events performed during the reporting period.
Maintenance Vents - $63.655(9)(13)
There were no deviations from the limits at 63.655(g)(6Xi) during the reporting period.
Changes in Information Provided in a Notification of Compliance Status (NOCS)
Report - $63.655($(14)
The following table serves as an update to applicability for certain "storage vessels," as
defined within 40 CFR 63.641, at the refinery and classifies each either as a Group I storage
vessel or Group 2 storage vessel. It also serves as updated documentation of regulatory
applicability. This list supersedes previously-identified Group I or Group 2 storage vessel
applicability for the tanks identified below.
