The completed application form signed by Boyd Davis and dated May 6, 2025, which formally designates: Boyd Davis, P.E., as the Professional Engineer (PE) having direct responsibility for the design of the entire water system, and Boyd Davis, P.E., as the PE responsible for oversight of the hydraulic analysis of the entire water system. Please note the following restrictions to this approval: 1. This approval does not apply to booster pump stations. 2. If minimum separation between water and sewer lines cannot meet the requirements in R309-550-8, an exception must be obtained prior to water line construction. This Approval expires on June 30, 2025. You must notify the Division if there is a change to standard installation drawings and specifications or the designated PE for design or hydraulic analysis during the term of this Approval. To continue using standard installation drawings and specifications for water transmission and distribution lines after the expiration date, you must submit a complete renewal application to the Division. Please maintain a copy of this letter with your permanent records for future reference.
