Utah Department of Environmental Quality 285 W Tabernacle St. STE 208 Saint George, UT 84770 Subject: Operating Permit Request: Open Sky Project #15287 – System # 27-108 Dear Paul Wright, Attached is the check list for an operating permit that finds applicable items acceptable. I certify that all changes made necessary during construction were in accordance with applicable Utah Administrative Code, R309-500 through R309-550. The Contractor completed the items needed to comply with the code. A copy of the as-builts have been sent to Bygnal, the owner. The following is a list of changes from the approval of project 15287: #1. Abandonment of the 550-gallon Norwesco tank (identified as ST003 in the Division’s database) located on the upper mesa and the two 1.5” water lines that were above ground going up and down the steep rocky terrain. The water lines were dug up where they came out of the ground, cut, connected back together, and buried. All water lines in use are buried underneath the ground. #2. The existing booster pump is located in an enclosure with drainage and pressure gauges (identified as PF002 in the Division’s inventory). This is a 2-horsepower pump with 17 gallons per minute (gpm) capacity that pulls water from the existing system that used to deliver water to the 550-gallon tank on the top of the mesa. This booster pump will stay and transition to a secondary backup booster pump for the new on-demand pump installed in the utility shed adjacent to the identified rental camp with low water pressure. A new plumbing diagram will achieve this (Please see Existing water booster pump in box enclosure detail), including a bypass line and a 60/40 pressure switch to control this pump to maintain water pressure in the 22-gallon bladder tanks located in the utility shed adjacent to the rental unit, in case the primary booster pump fails. #3. Installed is a new 2-horsepower booster pump with a 22-gallon-per-minute (GPM) capacity in the utility shed adjacent to the rental unit with low water pressure. Additionally, two 22-gallon fiber- wound pressure tanks are installed inside the enclosure. This system will be the primary booster pump for this rental unit. A new Inventory identification number will be needed from the division. Please see the water details on the submitted plan. #4. We had a local fabrication company weld shoebox lids for the 5000-gallon and 550-gallon tanks (located in the well building near the water treatment plant). We have them and they are ready to be installed on the two tanks. However, distribution lines without booster pumps may be placed into service prior to submitting all of the items if a professional engineer (PE) responsible for the entire water system has been identified to the Director and has received items 1 through 4 below. [In this case, a public water system will submit all items needed to obtain an Operating Permit for each distribution system project after the new water lines have been placed into service as determined by the water system’s designated PE.] ☒ 1. Certification of Rule Conformance by a PE that all conditions of Plan Approval have been accomplished, and if applicable, changes made during construction were in conformance with rules R309-500 through 550 ☒ 2. As-built or record drawings incorporating all changes to approved plans and specifications, unless no changes were made to the previously submitted and approved pre-construction drawings ☒ 3. Confirmation that as-built or record drawings have been received by the water system ☒ 4. Evidence that bacteriological samples have been collected to verify proper flushing and disinfection in accordance with the appropriate ANSI/AWWA Standards: ☒ ANSI/AWWA C651-14 AWWA Standard for Disinfecting Water Mains Two consecutive sample sets at least 16 hours apart, none positive (every 1200 feet, end-of-line, each branch, etc.) ☐ ANSI/AWWA C652-11 AWWA Standard for Disinfection of Water-Storage Facilities One or more samples, none positive ☐ ANSI/AWWA C653-13 AWWA Standard for Disinfection of Water Treatment Plants Two consecutive samples per unit, none positive, no less than 30 minutes apart ☐ ANSI/AWWA C654-13 AWWA Standard for Disinfection of Wells Two consecutive samples, none positive, no less than 30 minutes apart ☐ 5. Water quality data, where appropriate [Guidance: Include appropriate raw and finished water data that demonstrate satisfactory performance of the new treatment facility.
