Booth, General Manager Date Non-Proprietary New Proprietary Title Change Restricted Information Revision Safeguards Information Rewrite Sensitive Security Information Cancellation Clive Facility Specific Work Instruction CL-SH-WI-609-02 Rev 3 Mobile Equipment Process for Conducting Pre-Shift Inspections Non-Proprietary Page 2 of 6 1 Purpose 1.1 The purpose of this work instruction is to provide details on the site process used to inspect mobile equipment prior to each shift. 2 Inspection Forms 2.1 The following Pre-Use Inspection forms will be utilized at the Clive site: • CL-SH-WI-609-02-F1 Equipment Vehicle Daily Inspection Checklist. • CL-SH-WI-609-02-F2 Aerial Lift Pre-shift Inspection Checklist. 2.2 Contractor equipment shall be inspected prior to operation at the Clive site using the following form: • CL-SH-WI-608-02-F2 Contractor Equipment Inspection Form 2.3 Rental equipment shall be inspected prior to operation at the Clive site using the following form: • CL-SH-WI-608-02-F3 Rental Equipment Inspection Form 2.4 Completed Pre-use Inspection Forms, Contractor Equipment Inspection Forms, and Rental Equipment Inspection Forms shall be turned in to the Maintenance Department and retained for a minimum of one year. 2.5 A random audit of Pre-use Inspection Forms, Contractor Equipment Inspection Forms, and Rental Equipment Inspection Forms will be conducted by the Health & Safety Department annually. 3 Process Flowchart 3.1 The following is a process flowchart, beginning on the next page, shall be used as a guide for the paperwork flow of pre-use inspections: Clive Facility Specific Work Instruction CL-SH-WI-609-02 Rev 3 Mobile Equipment Process for Conducting Pre-Shift Inspections Non-Proprietary Page 3 of 6 Operator performs pre-use inspection Pass or Fail? Operator tags out equipment Operator notifies supervision and contacts maintenance department Operator signs pre-use inspection form, places in proper location on the equipment, and begins work NO YES Can issue be resolved immediately? Does an issue arise during the shift?
