Pursuant to Titte V Operating Permit 4500005004 issued to Lhoist North America of Arizona,
lnc. (Lhoist), Lhoist is reporting two permit deviations as required by condition 1.S.2.c. This permit
condition requires the prompt (written within 14 days) notification of att permit deviations; the
probabte cause of the deviations; and corrective or preventative actions. The information required
for the two deviations is in the tabte betow.

Lhoist staff was not
aware of this
requirement. Part of
the issue is that
tenure of the site
staff is less than a
year. The tenure of
Regionat Env Mgr is
about 15 months.
Lhoist staff inctuding the
Regionat Env Manager
and Responsibte officiat
are aware and
committed to meeting
this requirement. lt has
been added to a
compliance tracker.
