Procedure: Applying for a New Specific License (LPR)
1. Access the Portal
• Navigate to the portal: https://deqorg.utah.gov/s/login/?ec=302&startURL=%2Fs%2F
• Login with your credentials or register if you're a new user.
2. Navigate to the License Application Section
• Once logged in, click Manage LPRs from the left-hand ribbon.
• Select the option to Apply for a New License, Permit, or Registration (LPR).
3. Select or Add a Facility
• Check the dropdown for your existing facility.
• If your facility isn’t listed: Follow the prompts to create a new facility.
o Ensure you have the necessary permissions to add a facility.
4. Complete Application Questions
• Program Selection:
o Choose Radioactive Materials as the program.
o Choose the Radioactive Materials Specific License sub-program:
o Choose the appropriate license type from the Classification Types then click Save
and Next.
o Carefully read the Instructions. Download any necessary documents from links in
the Instructions.
o Make note of the Application Name. You will need this information to communicate
with RAM Program staff regarding your application.
o From this point on in the application process users can click Save and Exit to save
their application. Users should not proceed beyond the Instructions page until they
are certain that they have collected all the necessary information for their
application and prepared all the necessary documents.
o After making certain that they have collected all the necessary information for their
application and prepared all the necessary documents users should click Save and
Next.

• Input Facility Details (fields with a red asterisk are required):
o Enter Business Name registered with the Department of Commerce Division of
Corporations and Commercial Code.
o Facility Mailing Address
o Address of Location of Use: If there is no physical address, provide a description of
the location of use by clicking Add Description.
• Device and Source Information: Ensure device and source records are kept separate.
• Temporary Job Site: If the material will be used at a temporary job site, indicate this by
interacting with the checkbox.
5. Review and Proceed:
• Carefully review the information provided and proceed through each of the application
questions.
6. Enter Contact Information
• Provide the contact details of the individual responsible for the application.
• You an either select an existing contact or create a new one.
• Complete the contact form as prompted.
7. Assign Radiation Safety Officers (RSO)
• Click Add RSO to assign a Radiation Safety Officer and any Associate RSOs.
• You can either select an existing contact or create a new one.
• Ensure all required fields are completed for each contact.
8. Acknowledge the Authorized User Statement
• Check the box to acknowledge the Authorized User Statement.
• If not checking the box please enter the information for each authorized user.
• On the Application Documents page upload any required information from the applicable
NUREG.
9. Sealed Sources
• If the LPR includes sealed sources:
o Click Add a Source.

o Select Source details from the associated drop-down menu
o Provide the following details:
• Source type (e.g., Cs-137, Am-241, Ir-192)
• Source manufacturer
• Source model
• Chemical/physical form (select from dropdown)
• Maximum activity per source
• Unit of activity (mCi or MBq)
If the sealed sources are part of a device after completing the source information, select
add device and complete the information there.
• If the LPR does not include sealed sources: Click the No button and proceed to the next
question.
10. Purpose of Radioactive Material
• Fill in the purpose for which the radioactive material will be used.
11. Save and Proceed
• Click Save & Next to continue.
• Upload any required supporting documents. At least one document must be uploaded to
proceed.
12. Certify the Submission
• Check the Attestation Box to certify that the submission is accurate and complete.
• Click Save and Next.
13. Review Application Information
• Review the application on the summary page. To correct any information that is incorrect
click the name of the section, such as Application Questions, from the menu on the left to
return to that section and correct the information.
• Click Save and Next to move to the payment page.
14. Payment
• Submit payment as required.

• When payment Status changes to Paid the Submit button will be enabled. Users MUST
click the Submit button to complete the application process.

Procedure: Applying for a New General License (LPR)

o From this point on in the application process users can click Save and Exit to save
their application.
• Input Required Details (fields with a red asterisk are required):
o Enter the name of the Contact Person for the General License. If the contact person
is already in the database the contact person information can be found by clicking
in the Select Contact field and choosing the name from the drop down list.
o Enter the Physical Address for the General Licensee.
o Enter the required information for General License Devices by clicking the radio
button by Yes, the clicking Add a Source. After entering the source information

enter the device information by clicking Add New Device or search for an existing
device. Multiple sources and devices can be entered.
o Click Save and Next after entering all required information.
5. General License Devices
• Click Add a Source
o Select Source details from the associated drop-down menu
o Provide the following details:
• Source type (e.g., Cs-137, Am-241, Ir-192)
• Source manufacturer
• Source model
• Chemical/physical form (select from dropdown)
• Maximum activity per source
• Unit of activity (mCi or MBq)
After entering the source information enter the device information by clicking Add New
Device or search for an existing device.
6. Save and Proceed
• Click Save and Next to continue.
7. Application Documents
• This step is optional. Upload any supporting documents if necessary.
• Click Save and Next.
8. Certify the Submission
• Check the Attestation Box to certify that the submission is accurate and complete.
• Click Save and Next.
9. Review Application Information
• Review the application on the summary page. To correct any information that is incorrect
click the name of the section, such as Application Questions, from the menu on the left to
return to that section and correct the information.
• Click Save and Next to move to the payment page.

10. Payment

Procedure: Amending an Existing License (LPR)
1. Access the Portal
• Navigate to the portal: https://deqorg.utah.gov/s/login/?ec=302&startURL=%2Fs%2F
• Login with your credentials.
2. Navigate to the Issued LPRs Section on the Portal Dashboard
• Scroll through the table to find the license you wish to amend.
• Click Request for Amendment in the Action column on the right.
3. Make Necessary Changes
• After clicking Request for Amendment the user will be on the Application Questions page.
The fields will be completed with previously entered information. Users can change the
information by clicking the pencil icon to Edit or the trashcan icon to delete in the
Edit/Delete column.
• Add or Modify Authorized Users (RSO/ARSOs/AMPs/ANPs/AUs):
o Click Add Authorized User and select the type of user (e.g., RSO, ARSO, AMP, ANP,
AU).
o Enter or update the user’s information.
o Ensure all required fields are filled (name, qualifications, etc.).
o Attach any supporting documents at the Application Documents page
• Add or Modify Sources/Devices:
o If applicable, click Add Source to include new sources, or modify existing source
information.
o Provide details (type, manufacturer, model, chemical/physical form, activity, etc.).
• Once all changes have been made click the Save and Next button.
5. Application Documents, Save and Next

• Upload any required supporting documents including documents for amendments that
were not addressed on the Application Questions page. This is not required if no
documents are needed for the application.
• Click Save & Next to proceed.
6. Certify, Review, and Submit
• Check the Attestation Box to certify the accuracy of the amendments. Click the Save and
Next button.
• On the Summary page review the information that has been entered. To correct any
information that is incorrect click the name of the section, such as Application Questions,
from the menu on the left to return to that section and correct the information. Click the
Save and Next button.
7. Payment and Submission
• If this is a specific license payment is not required for an Amendment application. Click the
Submit button to submit the application.
• If this is a general license payment is required for an Amendment. Follow the payment
instructions found in the new license application section.
8. Confirmation
• Once submitted, you will receive confirmation of the amendments.
• Any further instructions or document uploads will be prompted as necessary.

Procedure: Termination of an Existing License (LPR)
1. Access the Portal
• Navigate to the portal: https://deqorg.utah.gov/s/login/?ec=302&startURL=%2Fs%2F
• Login with your credentials.
2. Navigate to the Issued LPRs Section on the Portal Dashboard
• Scroll through the table to find the license you wish to terminate.
• Click Request for Termination in the Action column on the right.
3. Make Necessary Changes
• After clicking Request for Amendment the user will be on the Application Questions page.
The fields will be completed with previously entered information. Users must enter
information into all the fields because they are all required. The License Number field will
be automatically filled in.
• Once all the fields have been completed click the Save and Next button.
5. Application Documents, Save and Next
• Please provide any additional remarks for information necessary by uploading documents
on the Application Documents page.
• Upload any required supporting documents. This is required.
• Click Save & Next to proceed.
6. Certify, Review, and Submit
• Check the Attestation Box to certify the accuracy of the amendments. Click the Save and
Next button.
• On the Summary page review the information that has been entered. To correct any
information that is incorrect click the name of the section, such as Application Questions,
from the menu on the left to return to that section and correct the information. Click the
Save and Next button.
7. Payment and Submission
• Payment is not required for a Termination application. Click the Submit button to submit
the application.
8. Confirmation
• Once submitted, you will receive confirmation.
