EVALUATION: With the exception of sources that only monitor for opacity, quarterly
report memoranda will only address the findings of the quarterly report.
Approval Order and Title V permit conditions addressing continuous
emission monitoring requirements will be addressed in Clean Harbors’
relative accuracy performance specification test review memorandum.

SOURCE INSPECTION
SUMMARY EVALUATION: Clean Harbors CO, O2, and THC monitors are MACT Channels. Clean
Harbors maintains its CEMS as required in R307-170 and 40 CFR 60.1.
The SEDR was submitted on time.
CURRENT
RECOMMENDATIONS: All deviations appear to be a result of breakdown or process related
problems that do not warrant issuing a violation. Periods of excess
emissions that were recorded by the CMS in service on each point source
during this reporting period are summarized in the SEDR and described
in Pivot Table 400 “Excess Emissions Review.”
Clean Harbors was in compliance during this reporting period. No
further enforcement actions should be taken.
Clean Harbors’ NSPS monitoring system meets the performance
standards of 40 CFR 60, Appendix B Performance Specifications.

VIOLATOR (HPV): No
RECOMMENDATION
FOR NEXT INSPECTION: Check DAS for daily zero and span drift.
Check the QC program to ensure that monitor drift is being tracked and
that the source can identify excessive drift.

Dear Mr. Bird.
In accordance with Utah Administrative Code (UAC) R307-170-9.40 CFR 63.10(e), and Clean
Harbors Aragonite, LLC (CHA) Title V Permit. CHA is submitting the l't Quarter 2025 Excess
Emissions Report (EER). A copy olthis report can be found on the enclosed CD. This report has
also been sent via email to Mr. Rob Leishman of the Utah Division of Air Quality (UDAQ) on
April14,2025.
Additionally, as required by 40 CFR 63.1206(c)(3)(ii), this letter is serving as the summary of
the investigation and evaluation of ten (10) exceedances of an emission standard or operating
requirement during a malfunction event.
During this quarter, there were not ten or more exceedances ol THC, SO2 or NOx. CHA is
continuing efforts to watch for profiling issues that could cause potential THC, SO2 or NOx
cutoffs within the system.
In accordance with Permit Condition I.K and UAC R307-415-5d, I certiff under penalty of law
that this document and all attachments were prepared under my direct supervision in accordance
with a system designed to assure that qualified personnel properly gather and evaluate the
information submitted. Based upon my inquiry of persons or persons who manage the system, or
those persons directly responsible for gathering the information submitted is. to the best ol my
knowledge and belief. true, accurate and complete. I am aware that there are significant penalties
for submitting false information, including the possibitity of imprisonment for known violations.
